Necrosis of the head of the femur (after decompression bone grafting surgery, within 3 years or before total hip replacement); xvi. Mental disorders. Except for patients with leukemia, malignant tumors of various systems of the body and chronic renal failure who have been on dialysis, patients with other diseases must be discharged from hospitals after inpatient systematic treatment, and their condition meets the criteria for the identification of outpatient co-ordination of medical insurance before they can participate in the identification. When discharged from the hospital, insured patients who meet the criteria for the identification of integrated diseases need to go to the hospital medical insurance office to get and fill in the Application Form for the Identification of Integrated Diseases and the Specialized Medical Record Booklet, and at the same time make a copy of the relevant diagnostic basis of the disease. Within one month after discharge from the hospital, the patient should send the inpatient and outpatient medical records, the Application Form for Recognition of Comprehensive Disease Types, the Specialized Medical Record Booklet, the diagnostic basis of the disease (copy) and two one-inch photos to the unit. After the unit has affixed its seal on the Application Form for Recognition of Comprehensive Disease Types, the unit will submit the above information to the Municipal Health Insurance Office to apply for recognition of comprehensive disease types. Patients applying for the recognition of disease types will participate in the medical check-up organized by the Municipal Health Insurance Office according to the regulations, and the cost of the check-up will be borne by the individual.
