Abstract: Diabetes mellitus is a relatively common chronic non-infectious disease in China. However, the rate of awareness, treatment, and control of diabetes is relatively low in China at present, so the prevention and treatment of diabetes is a heavy and difficult task. Therefore, it is important to set health care goals for the elderly, develop and implement a simplified set of care programs, and ensure that the continuity of care can be maintained by various medical units with different caregivers in order to improve the care of diabetes mellitus in the elderly and the prevention and treatment of related complications, and to achieve continuity of care with temperature, breadth, and precision.
