Materials required for first application
p>1. Application report (including a brief description of the unit’s basic situation, application content and business capabilities related to the application content);
2. "Construction Project Environmental Impact Assessment Qualifications" Application Form";
3. Proof of legal person qualification (original and duplicate copy of business license or certificate of corporate legal person) and copy of the articles of association of the legal person;
4. Fixed Proof of assets (copies of purchase invoices or other valid certificates for real estate, equipment, etc.);
5. Proof of workplace (property certificate, lease contract or other valid certificates);
6. Copies of labor relationship certificates for full-time environmental impact assessment technicians (the issuance time should generally not be earlier than six months before the application time):
(1) Institutional establishment staff can submit it to the personnel department of the unit or Proof of personnel relationship (formally in preparation) provided by the superior personnel department with the power of personnel management; personnel employed by public institutions can submit a list of vouchers or certificates issued by the social insurance management agency for participating in social insurance (pension, medical, unemployment) in the unit and Labor contract;
(2) Enterprise personnel can submit a list of vouchers or certificates issued by the social insurance management agency for participating in social insurance (pension, medical, unemployment) in their unit and a labor contract;
(3) Re-employment or employment personnel after retirement can submit a retirement certificate and a re-employment certificate or labor contract issued by the unit.
(4) Those who are unable to provide the above materials due to special circumstances should provide a detailed description of their labor relations and attach other valid certificates.
7. Copies of academic certificates of full-time environmental impact assessment technicians, nuclear safety engineer registration certificates, etc.;
8. Environmental impact assessment internal management and quality assurance system and other relevant documents;
9. Proof of personal performance (the first page of the archived version of the environmental impact report prepared by the environmental impact assessment engineer related to the scope of the applied assessment, the personnel list and a copy of the approval);
10. Resume and copy of ID card of the legal representative;
11. Original materials for the first or re-registration of environmental impact assessment engineers (for specific requirements, please see the Environmental Impact Assessment Engineer Professional Qualification Registration Management relevant regulations).
Note: The above-mentioned materials in item 1 are bound separately, materials in items 2-10 can be bound together, and materials in item 11 are sorted separately according to relevant requirements.
