Paper materials to be submitted by importing users.
(1) To apply for the Automatic Import License of Mechanical and Electrical Products, the following materials shall be provided:
1. Original application form for import of mechanical and electrical products with official seal;
2. A copy of the business license of the importing user;
3. A copy of the goods import contract;
4. If the import is entrusted, an agreement on entrusted import shall be submitted;
5. In any of the following circumstances, the following materials shall also be provided:
(1) For importing mechanical and electrical products of medical devices, a valid medical device registration certificate of the imported products and a medical device business license of the importing unit (photocopy) shall be provided;
(2) Import the electromechanical products of CNC machine tools, and provide the technical parameter list of imported CNC machine tools (the original is sealed);
(three) the import of tobacco equipment and mechanical and electrical products, the imported equipment must be in the annual schedule compiled by the State Tobacco Monopoly Bureau, and the application form must be stamped with the newspaper of the Mechanical and Electrical Department of the Tobacco Monopoly Bureau;
(4) The import of mechanical and electrical products of optical disc production line equipment shall be approved by the local press and publication department (original), and the optical disc industry business license (copy) shall be submitted for the imported users;
(5) When importing mechanical and electrical products purchased through international bidding, the Notice of International Bidding Evaluation Results issued by the international bidding agency shall be provided;
(6) Where used mechanical and electrical products are imported, the pre-inspection report of the imported products issued by the inspection and quarantine institution authorized or licensed by the AQSIQ shall be provided;
(seven) mechanical and electrical products imported without bidding1000000 yuan or more, provide the certificate of private enterprise issued by the industrial and commercial bureau of the place of registration, and affix the file inquiry seal or official seal;
(eight) other materials required by the Ministry of Commerce.
(2) To apply for import license of key used mechanical and electrical products, the following materials shall be provided:
1. Apply for an import report (introducing the general situation of the enterprise and the purpose description of the used mechanical and electrical products to be imported);
2 mechanical and electrical products import application form (indicate the year of manufacture in the specification column of the application form, and affix the seal of the importing user);
3. A copy of the business license of the importing user (with the seal of the importing user's unit required);
4. Certification materials of the production year of key used mechanical and electrical products applied for import;
5. The applicant shall provide a description of the equipment status;
6. If the import is entrusted, the entrusted import materials shall be submitted;
7. Where other relevant laws and administrative regulations have specific provisions on "importing users" or "uses of imported goods", relevant certification materials that "importing users" or "uses of imported goods" conform to the provisions of the state shall be submitted.
