What qualifications are required for medical devices to enter the hospital for the record

1. The bidding unit must provide the following qualification documents (provide copies, and provide the original for inspection):

① "Business license for enterprise legal person"

② "Organization Code Certificate"

③ "Tax Registration Certificate" (including national tax, local tax tax registration certificate)

④ "Medical Device Business Enterprise License" or "Medical Device Manufacturer's License"

⑤ "Medical Device Registration Certificate", "Medical Device Product Manufacturing License"

⑥ "Medical Device Quality Management System Certification", "Medical Device Product Certification Certificate"

D. Other relevant information

--The bidding for imported medical equipment Enterprises, must also provide the following qualification documents:

A. Imported products of the distribution company in the domestic legal registration certificate.

B. Equipment import license, customs declaration information and commodity inspection certificate (if the bidding can not be provided, the equipment must be provided at the time of installation of the item, otherwise, the equipment is deemed unqualified acceptance.)

C. Other relevant information.

2. Bidder qualification requirements

Enterprise legal representatives to participate in the bidding, must hold their own identity cards and photocopies; non-corporate legal representatives to participate in the bidding, the bidder must hold a legal representative of the power of attorney (stamped with the official seal of the enterprise) and their own identity cards and photocopies.
