1. Organizational Leadership

(1) Municipal Leadership Organization

It has an examination team and an inspection team.

(2) District and county organizational structure

Each district and county needs to establish a corresponding leadership group with a number of working groups responsible for organizing and implementing the physical education examination work in the district and county.

2. Examination objects and examination registration

(1) Examination objects

Anyone who participates in Xi'an's 2015 junior high school graduation examination (i.e., promotion to ordinary high school, Candidates of vocational high schools and secondary vocational schools (hereinafter referred to as the high school entrance examination) (including former students, candidates who studied abroad and returned to our city to take the high school entrance examination and all junior high school graduates) must take the unified physical education examination; anyone who does not take the unified physical education examination without reason Candidates who pass the examination are not allowed to advance to a higher school.

(2) Examination registration

The registration for the physical education examination is carried out simultaneously with the registration for the middle and upper grades, with unified numbers. Candidates take the unified physical education examination in the registration district and county. Candidates can choose and apply for the unified physical education examination items during the online registration process. After the registration information is confirmed, it cannot be modified.

3. Exam content, performance evaluation and examination methods

The physical education examination consists of daily assessment and unified examination, with a full score of 50 points. The full score for the usual assessment is 15 points, and the full score for the unified examination is 35 points. The test results are presented in four grades: A, B, C, and D. 50-45 points are excellent (A), 44.9-37.5 points are good (B), 37.4-25 points are passing (C), and 24.9 or less are considered passing (C). It is unqualified (D). The raw scores are included in the total score of the high school entrance examination.

(1) Daily assessment

1. Daily assessment for fresh graduates: Daily assessment is 5 points per academic year, and the full score for three years is 15 points. Among them, the physical education class score is 3 points per school year, with a full score of 9 points in three years; the "National Student Physical Health Standards" test score is 2 points per school year, with a full score of 6 points in three years. The school is responsible for organizing and implementing the daily assessment. Each school must report the daily assessment results of fresh graduates to the district and county admissions examination agencies before April 3.

2. Daily assessment of previous students: 80 (i.e. 12 points) of the full score of the usual assessment will be directly credited by the district and county admissions examination agencies.

3. For fresh graduates studying in other places or across districts and counties in our city, the schools where they attend will issue certificates of their usual assessment scores and submit them to the district and county admissions examination agencies for review and recording of their scores.

(2) Unified Examination

The unified examination is organized and implemented by the admissions examination agencies of each district and county, and the time is: April 5 to April 30, 2015.

1. Examination items:

The full score for each item in the unified examination is 100 points.

2. Examination method: In accordance with the description of the examination method stipulated in the "Xi'an 2015 Uniform Physical Education Examination for Junior High School Graduation Examination Implementation Rules" and the "Xi'an 2015 Unified Physical Education Examination Position Workflow" (hereinafter referred to as Workflow) execution (see attachments 1 and 2). The conversion formula is: total score 35 points = (score for item 1, score for item 2, score for item 3, score for item 4) ÷ 4 × 35

4. Test equipment: except for volleyball skill items All exams are conducted using electronic test equipment.

IV. Examination Regulations and Requirements

(1) Examination Agency

An examination agency set up in each district and county is responsible for the unified physical education examination. organizational implementation.

The examination agency shall have one chief examiner, who shall be the director in charge of the district and county education bureau, and one or two deputy chief examiners, who shall be the directors and deputy directors of the district and county admissions examination agencies (see Appendix 5 for responsibilities). 1. Responsibilities of the assessment team: set examination procedures; standardize the layout of the examination venue according to the requirements of the examination items, strictly inspect the examination equipment and facilities; provide reasonable job division and solid business training for the assessors; organize and implement Examination work for each project; handling examination issues during the examination process; ruling on and stopping candidates' disciplinary and irregular behaviors. The assessment team leader shall be a physical education teacher with a senior professional title.

The assessment team consists of an inspection team and an examination team, whose responsibilities are:

(1) Inspection team: Check whether candidates’ clothing complies with regulations; urge candidates to prepare for activities; Candidates will be grouped into groups as required (the groups must be grouped according to the principles of the same class, same gender, and same project); the candidates who have been inspected will be brought into the examination venue in line according to the work process.

2. 3. (2) Test center and examination room setting

1. 2. Examination room settings: The unified examination venues and venues must be free of safety hazards, and the inspection venues, waiting areas, examination venues, toilets, etc. can be arranged reasonably and loosely, and between each venue (including each project venue) between) with safe passages. (3) Use of test instruments and other equipment

1. All items in the unified examination except the volleyball skill items are tested using electronic testing instruments.

2. During the examination process, all items of the unified examination must use uniformly equipped equipment for video monitoring and recording (video recordings must be kept for more than half a year).

3. After each item of the exam using electronic testing equipment is completed, the "Unified Examination Result Report Form" will be printed out to the candidates immediately.

4. Dedicated monitoring and inquiry terminals are set up in the examination center for use by supervisors and staff. The usage rights of the terminal must be set to only query, modify and delete any data.

5. Before the exam, all districts and counties must contact the instrument manufacturer according to the unified examination work requirements, complete the setup of the examination system software in a timely manner, and maintain, debug and test the examination instruments. Any instrument that has not obtained the test qualification report shall not be put into use.

6. Before March 30, all test centers in the city must conduct a complete mock test of the entire examination process for all items of the unified examination. Ensure that all examination equipment is operating normally and that there are no safety hazards in all examination locations, and the simulated examination results shall be reported to the Municipal Education Examination Center in writing before April 3.

(4) Fill in the "Admission Ticket"

1. The admissions examination agencies in each district and county will print and issue the "Admission Ticket" according to the prescribed time (the printing software and format shall be in accordance with the Municipal Education Examination Center's unified template). Schools at each registration point must arrange for dedicated personnel to carefully check the name, admission ticket number, unified examination items, barcodes, photos and other information on each candidate's "Admittance Ticket" (if any incorrect information is found, promptly feedback and correct it to the district or county admissions examination agency), confirm Once it is correct, it will be stamped with the seal of the school or the district or county admissions examination agency, and a dedicated person will be assigned to strictly manage and preserve the "Admission Ticket" to prevent it from being lost.

2. Schools at each registration site should issue the "Admission Ticket" to each candidate before the unified examination. Candidates must hold the "Admission Ticket" to participate in the unified examination.

3. During the process of filling in the unified examination results column of the "Admission Ticket", if there is a clerical error or an error in the results, it should be corrected in time and signed and sealed. When the "Admission Ticket" needs to be replaced, the assessment team leader must fill in the reason for the replacement and sign in the upper right corner of the invalid certificate, and attach the invalid certificate to the new certificate.

(5) Data Management

1. After completing all exams for each examination group, the collected "Admission Ticket" must be handed over to the score administrator for safekeeping in a timely manner. 》Must be sealed in the security cabinet in the security room. 2. Set operating permissions for unified examination instruments and work computers, and assign responsibilities to people. Information administrators must complete tasks such as computer transmission under the supervision of supervisory department personnel and performance administrators.

3. Computers with unified examination result data must be kept by more than two people during the unified examination period (including lunch and break times) and must not be taken out of the unified examination venue, and the computer must be operated in a safe manner. 4. All districts and counties must submit the combined results of the physical education examination to the Municipal Education Examination Center in the form of a CD before May 15. 5. After the unified examination is completed, each district and county must send the "Admission Ticket" of all candidates to the school as a unit. The admission ticket numbers are arranged in ascending order and kept in the district or county confidentiality room until half a year after the cultural class exam ends. The municipal examination supervision team (Municipal Education Bureau Supervision Office, Physical Education and Health Department, Municipal Education Examination Center) will conduct random inspections.

(6) Examination exemption and deferment

1. Examination exemption.

Candidates due to illness (serious diseases of the heart, lungs, liver, kidneys and other major organs), abnormal physical development, physical disabilities, or deformities (those that seriously affect their functions) can apply for exemption from the physical education examination.

2. Delay the test.

Candidates who are unable to take the unified physical education examination as scheduled due to sudden illness (or injury) or menstrual period may apply in writing to defer the exam. The exam can be postponed only after approval by the district or county admissions examination agency. (7) Avoidance system

(8) Information submission

(9) Pre-examination education

5. Work Requirements

(1) Strengthen organizational leadership and strengthen awareness of responsibility

The Municipal Education Examination Center and the district and county education bureaus have established work leading groups. (2) Strict management, strengthening supervision, and serious examination of style and discipline are the top priorities of sports examination work.
