One-stop" Surgery in Fuwai: A New Model of Cardiovascular Disease Treatment

With the development of China's economy and changes in people's lifestyles, cardiovascular diseases have become the number one killer of people's health. Many cardiovascular diseases, such as congenital heart disease, coronary heart disease, etc., in addition to drug treatment, need to be "invasive" means of intervention.

Surgical open-heart and catheterization interventions are currently the two most common "invasive" treatments. The former is effective, but the surgical incision is large and traumatic; the latter can be accomplished percutaneously, but there are still limitations on the patient's condition and a lack of efficacy on its own. Is it possible to have a "perfect" medical treatment that combines the advantages of the above two treatments into one, thus giving patients the benefit of minimal trauma? This is the purpose of the newly established "one-stop" surgery center at Fu Wai Cardiovascular Hospital in Beijing.

"One-stop" operating room: a new platform for cardiovascular disease treatment

"One-stophybridprocedures" include the following features:

1. Surgical treatment of heart disease is accomplished in an operating room that combines multiple imaging devices.

2. The treatment is performed by a surgeon after a thoracotomy using conventional or modified interventional devices.

3. There is a high degree of reliance on intraoperative imaging to guide surgical maneuvers and provide immediate evaluation of surgical outcomes.

"One-stop" surgery is a product of multidisciplinary integration, the cornerstone of which is the increasingly mature cardiac surgery, including the choice of hidden incisions and non-corporeal circulation examination, the application of large collection of technology; the core of its content is the use of interventional devices by the surgeon in a special operating room, under the guidance of real-time imaging, combined with the use of conventional or modified interventional devices, and the use of real-time imaging. real-time imaging guidance, combined with other conventional cardiac surgery to complete the treatment of heart disease.

"One-stop hybrid" surgery has a hybrid operating room that can perform both imaging and conventional cardiac surgery, so there is no need to transfer patients between the imaging department and the operating room multiple times, and all operations can be done in the same operating room, avoiding the risks of multiple anesthesia and transfers of patients. This avoids the risks associated with multiple anesthesia and patient transfers. More importantly, in such a "one-stop" operating room, the efficacy of the surgery can be evaluated instantly, thus guiding the implementation of the surgery.

The ideal "one-stop" operating room would have a wide range of imaging equipment, including a C-arm for cardiovascular imaging and a cardiac ultrasound system. However, it is difficult to find an operating room that has all of the above, even in the world's leading cardiac centers. The "one-stop" operating room established in Fuwai, which uses a full set of Siemens equipment, can simultaneously perform cardiovascular anesthesia, imaging, ultrasound, extracorporeal circulation and open-heart surgery in one operating room, which is the first "one-stop" operating room in China.

Advantages of "one-stop" surgery

FWH is currently preparing to implement the three major categories of diseases, basically covering the basic types of "one-stop" surgery, including "congenital atrial septal defects Congenital atrial septal defects under the wakefulness of trans-thoracic blockage surgery", "coronary heart disease multi-branch lesions combined with stent implantation of small incision bypass surgery" and "aortic aneurysm stent surgical implantation".
