Two? General requirements?
1?Enterprises should ensure that the application materials submitted are true and effective.
2?The application materials, except for certificates and licenses, should be in Chinese.
3?Enterprises should use A4 paper and print on one side without binding when preparing written application materials.
4?The part to be filled in or signed should be done with a pen or signing pen.
5?All application materials that need to be stamped should be provided with the original stamped.
6?When the applicant's mailing address, contact person, contact phone and fax changes should be notified in writing to the Telecommunications Equipment Certification Center.
7?Material requirements, if any changes, or the need to provide additional explanatory material, according to the then requirements.
Three? List of application materials?
2? Production companies do not have access to quality system certification or certification does not cover the scope of the application for access to the network of telecommunication equipment, you can apply for a system audit by the Telecommunications Equipment Certification Center to arrange for sampling and testing.
3?To apply for replacement of the network access license for telecommunications equipment, the following materials need to be submitted
3.1?Expiry of the license
3.1.1?Network access license (including trial approval) validity of the cumulative total of three years to apply for a new license, should be in the validity of the expiration of the three months before the submission of the application materials to the Telecommunications Equipment Certification Center, the certification of telecommunication equipment arranged by the Center for expiry of the replacement of the license inspection. 3.3?Expansion and renewal of license. Network license (including trial approval) has not expired, the product has undergone technical changes, should apply for expansion of the license. Enterprises will be A, I, J, K, O, U, V and other application materials submitted to the Telecommunications Equipment Certification Center, the Telecommunications Equipment Certification Center to arrange for the expansion of the license inspection.

Four? Specific requirements for each application material (corresponding to the material code in Table I and Table II)
1? A? Application Form

1.4?Requirements for filling in the columns in the application form
1.4.1? "Applicant Unit": the applicant unit must be an enterprise in the territory or a branch of an overseas enterprise registered in the territory. The name filled in should be consistent with the business license and official seal. Overseas enterprises can not be the application unit.
1.4.2? "Producer": regardless of whether the producer in the territory or abroad, fill in should be consistent with the license, official seal, quality system certification certificate of the enterprise registered in the territory or abroad.
1.4.3? "Nature of the enterprise": should fill in the type of enterprise of the production enterprise. For domestic enterprises to fill in according to the legal person business license, for overseas enterprises directly check the "overseas enterprises".
1.4.4? "Type of enterprise", "legal representative", "number of enterprises" and other columns, should be filled in the application unit.
1.4.5? "Origin": should fill in the application for the final origin of the equipment into the network and indicate the province and prefecture-level cities. Manufacturers outside the country only need to fill in the name of the country or region.
1.4.6? "Based on the standard": should fill in the model of telecommunication equipment used in the main technical standards, the name of the number.

2? B? Agency power of attorney
2.1?Overseas manufacturers in the application for equipment into the network, should be registered by its branch in China as the application unit to submit applications, this time do not need "agency power of attorney". When there is no branch in China, or there are special reasons, need to entrust other enterprises as the application unit, should be entrusted with after-sales service capabilities of the enterprise as the application unit to submit an application, this time should provide the agency power of attorney.
2.2 The power of attorney for the agency shall be the original signed by the legal representatives of both the authorizing party and the authorized party, with the official seals of both parties and the date of signing.
2.3?The power of attorney for the agency shall specify the name and model of the agency equipment in detail.
2.4?The application should be submitted within 3 months of the signing date.
3 C? Entrusted processing agreement
3.1?The content of the agreement should include the name of the equipment to be processed, based on the standard, the two sides should be clearly defined product after-sales service responsibilities.
3.2?There should be the signature of the legal representatives of both parties, the date and stamped with the official seal of both units.
3.3?Shall submit the original or a copy of the official seal of both units.
3.4?Application for network access should be made three months before the expiration of the agreement.
4 E? Business license of enterprise legal person
4.1?A copy of the business license with the confirmation seal of the licensing agency should be provided (not to be photocopied again after the confirmation seal is stamped) or the original copy of the business license should be presented to the center for confirmation, or the copy of the business license can be photocopied by the enterprise itself, but it should be indicated on the copy that the copy is the same as the original, signed by the legal representative of the enterprise, and stamped with the seal of the enterprise and the time of copying. seal and the time of copying. 4.2?The business scope of the business license of the enterprise shall include the production of the products applying for network access.

5.1?The basic profile of the enterprise: should be introduced to the enterprise's establishment time, the number of people, plant area, registered capital, fixed assets, production capacity and the enterprise's main products and other information.
5.2?Production conditions: should introduce the enterprise in the production of the application for access to the product, the use of equipment, write the name of the production equipment in use, model, quantity, origin and other information, you can use the form of a list of notes.
5.3?Instrumentation: should be introduced in the inspection of the application of the enterprise in the network of products, the use of instrumentation, write the name of the instrumentation in use, model, quantity, origin and other information, can be indicated in the form of a list.
5.4?Quality assurance system: should be detailed on the quality assurance system of the company's establishment of time, running time, operation, the company's current organizational chart, when to pass what kind of certification and through the certification of the annual supervision of the situation and other content.
5.5?After-sales service: should be described in detail in the sale of the product into the network of after-sales service, including after-sales service commitments or effective after-sales service flow chart. Belonging to the state regulations to implement the three packages (package repair, package replacement, package refund) of products, should provide the implementation of the three packages of commitment and indicate the time limit and conditions of the three packages. The enterprise's commitment should be consistent with or higher than the national regulations.

5.6 When applying for mobile phone network access, the format of the enterprise profile refers to the "Mobile Phone Manufacturer Profile Template".
5.7?All the above materials submitted by the enterprise shall be stamped with the official seal of the enterprise.
6 G? Quality system certification
6.1?ISO9000 or TL9000 certificate. Should provide the original and a copy of the official seal of the enterprise, or a copy of the certificate issued by the certification body stamped with the seal of proof (after the confirmation seal shall not be copied again).
6.2?The scope of certification in the certificate should include the production of the products applied for into the network.
6.3?If the certification is not in Chinese, a Chinese translation should also be provided and stamped with the official seal of the applicant.
6.4?The application should be submitted three months before the expiration of the validity of the certification certificate.
7 I Equipment Introduction
7.1?Terminal equipment
7.1.1?Equipment Overview: It should state the name and model of the equipment applying for access to the network and be consistent with the name and model filled in the application form. Wireless terminal equipment should indicate the model and manufacturer of the wireless transceiver module used (Exhibit 9).
7.1.2?The standard on which the equipment is based.

11 N? Radio transmitting equipment type approval certificate
11.1?The original and one copy with the official seal of the enterprise should be provided.
11.2?The model number in the radio transmitting equipment type approval certificate should be consistent with the model number filled in the application form.
12 O? Overall Technical Program
12.1?The specific content is prepared with reference to Article 7 of the "Regulations on the Administration of Expert Evaluation of Telecommunication Equipment for Network Entry" (Letter of Credit and Telecommunications [2001] No. 125).
12.2?It should be stamped with the official seal of the applicant organization.
13 P? test report
13.1?Specific content with reference to the "Telecommunications Equipment into the network expert evaluation management regulations" (letter [2001] No. 125), Article VIII preparation.
