Criteria for objects with income difficulties
1, destitute dependents: out-of-pocket expenses within the overall scope of medical insurance hospitalization, and assistance shall be given at the rate of 100%, with the maximum annual assistance of 50,000 yuan;
2. Urban and rural subsistence allowances: 70% of the out-of-pocket expenses within the overall scope of medical insurance hospitalization will be given assistance, with a maximum annual assistance of 50,000 yuan;
Standard of medical expenses for needy objects
1. Hospitalization assistance: When the out-of-pocket expenses within the overall hospitalization scope reach more than 30,000 yuan, 50% of the part exceeding 30,000 yuan will be given assistance, with the maximum annual assistance of 50,000 yuan.
2. Outpatient assistance: If chronic diseases such as uremia need long-term medication or serious diseases need long-term outpatient treatment, after being reimbursed by urban and rural residents' basic medical insurance or employees' basic medical insurance for chronic diseases, various supplementary medical insurance and commercial insurance, individuals will pay a fee to give single-disease assistance.
Rescue standard: patients will be rescued when their out-of-pocket expenses and fees reach more than 2,000 yuan, and 50% of those above 2,000 yuan will be rescued. When the relief fund is insufficient in 500 yuan, it shall be rescued in 500 yuan; Higher than 500 yuan, according to the provisions of the proportion of assistance, the maximum annual assistance of 50 thousand yuan. The cost of treatment can be calculated cumulatively within one year.
Special reminder: eligible recipients, out-of-pocket expenses in medical insurance coverage after reimbursement of basic medical insurance, serious illness insurance, various supplementary medical insurance and commercial insurance, can apply for assistance only if they meet the above conditions.

5. A copy of the first page and my page of the household registration book and my ID card;
6, the original fee receipt, the original insurance company claims, the original serious illness insurance compensation calculation.
