[In 2003, the average social wage of workers in our city was 13867.00 yuan× (1+10% )× 4.9% = 747.48], and the medical subsidy in 50 yuan increased by 797.48 yuan.
The following is the procedure.
1, receive and fill in the Registration Form for Flexible Employment in Changchun to Participate in Medical Insurance.
2. I carry the completed registration form, the original and photocopy of the ID card, the original and photocopy of the household registration book (home page and my page) and two recent bareheaded photos (one inch color). Migrant workers also need to hold temporary residence permits issued by the public security department and employment contracts issued by the labor department. The renewal personnel shall carry the ID card, the certificate of dissolution of the relationship, the original and photocopy of the medical insurance card.
3, more than the social wages of workers in the city as the payment base, according to the proportion of 4.9% of the payment to pay a year of hospitalization fund and large medical insurance 50 yuan, and pay the Changchun social security card fee 50 yuan.
4. Receive the social security card of Changchun City one month after the payment date (if the medical insurance card is made incorrectly due to the reasons for filling in the form or the mistakes in filling in the form, it is necessary to pay the social security card fee of Changchun City again).
