I. First-time Selection of Outpatient Coordinated Designated Medical Institutions
(1) Required Documents:
1. Identity card or social security card;
2. Application Form for Individual Selection of Outpatient Coordinated Designated Medical Institutions of Zhuhai City in duplicate.
(2) Procedures:
1. The insured person, with the required information, goes to one of the following places to go through the selection procedures: (1) Outpatient Coordination Designated Medical Institutions; (2) Neighborhood (Village) Committees; (3) Labor Security Offices; and (4) Social Insurance Administrators. In case of a proxy, the identity card or social security card of the proxy is also required.
2. The participant or the agent will fill in the selected application form and go through the selected procedures after bringing all the information to the above places.
II. Re-selection of Outpatient Coordination Designated Medical Institutions
(1) Required documents:
1. ID card or social security card;
2. Two copies of Application Form for Individual Selection of Outpatient Coordination Designated Medical Institutions in Zhuhai.
3, one of the valid supporting documents such as transfer order, labor contract, real estate certificate, household register, house rental certificate, etc.
(2) Procedures
Participants with the required documents to the new selected institution for the change.
(3) Precautions
After a participant selects a designated medical institution for outpatient coordination, in principle, no change will be made within the social security year. In case of job transfer or change of address, the change can be made at the newly selected organization. If a participant needs to re-select a designated medical institution for outpatient coordination in the next year, he should go to the new designated medical institution for outpatient coordination within 3 months before the start of the next social security year to apply for the change; if he fails to apply for the change, he is deemed to have continued to select the original institution.
