1. The health notice of the insurance application should be truthfully filled in.
2. The signature column of the applicant and the insured must be filled in by both the applicant and the insured. When the applicant applies for insurance for someone other than himself, the signature of the applicant on the insurance application must be the signature of the applicant himself. Anyone signing on behalf of the insurance contract will be invalid.
3. For health insurance products that need to be beneficiaries, the insurance policy has a beneficiary column. When filling in the beneficiary, you should fill in the beneficiary's real name, not the title.
4. All names should be exactly the same as those on valid certificates (resident ID card, military officer's card, household registration book).
