Is chronic disease management the same as chronic disease management?
Chronic disease management and chronic disease management are the same. It refers to the comprehensive, systematic and continuous management and intervention of patients with chronic diseases in order to control the progress of the disease, alleviate the pain and improve the quality of life. Chronic diseases usually refer to diseases with long course, slow progress, difficult treatment and poor prognosis, such as hypertension, diabetes, coronary heart disease and chronic obstructive pulmonary disease. Therefore, the goal of chronic disease management or chronic disease management is to help patients alleviate their pain, improve their quality of life, prolong their lives and reduce medical expenses through comprehensive, systematic and continuous intervention and management.
