China Bank lost its Shenzhen citizen health card. How to report the loss?

Citizen health card loss reporting is divided into ordinary health card and multifunctional health card.

1. Ordinary health cards are not allowed to report loss. Customers can re-open a new general health card through the networked hospitals in our city.

2. The balance of debit card account and card electronic cash account in multifunctional health card can be reported as loss, but card electronic cash account cannot. For specific provisions on loss reporting, please refer to the Administrative Measures for Great Wall Debit Card Business of Bank of China Limited.

(1) When the multi-functional health card is handled through the counter channel by debiting the B card for temporary loss reporting, the customer needs to fill in an application for personal loss reporting, provide the valid identity certificate of the cardholder, and try to provide supplementary information such as card number, card opening date, credit balance in the card, reserved address when opening the card, contact telephone number, etc. When customers apply for cancellation and temporary loss reporting, they must submit their debit cards and valid identity documents to the bank manager for relevant procedures.

(2) If the cardholder loses his multi-functional health card for various reasons, he can apply for a formal report of loss by debiting B, and he must fill in an application form for reporting the loss of individual customers, and provide the cardholder with valid identity documents to handle relevant formalities. After the official loss reporting period expires, the customer will take the card, and the customer himself will handle the relevant formalities with the personal customer loss reporting application and the cardholder's valid identity certificate. Card collection is not allowed. In principle, the cancellation of the formal loss reporting of IC cards in real-name registration system should be handled by the cardholder with valid identity documents and the original application for loss reporting.

(3) After the expiration of the official loss reporting period, the customer replaces the prefabricated card, and the customer himself holds the personal customer loss reporting application and provides the cardholder with valid identity documents to handle the relevant procedures for replacing the prefabricated card.

(4) After the official loss reporting expires, the customer will cancel the card. The customer himself holds an application for reporting the loss of an individual customer and provides the cardholder with valid identity documents to handle the relevant procedures for card cancellation.
