4. 1 license category application
Fill in the specific category of special equipment to be manufactured.

4. 1.5 stands for product (restricted range, typical product): the product whose application is restricted according to relevant licensing regulations, or the restricted range or product should be filled in, which should be clearly filled in; If there is no restriction, you can fill in its representative or typical product.

5. 1 opinions of the competent department of the unit
At present, if there is a competent department of the unit, the competent department of the unit shall put forward opinions and generally fill in "consent". If there is no competent department, you can fill in the applicant's superior management department, such as a company; If the applicant has been regarded as a completely independent entity and has no management department at any time, you can leave this column blank. However, the applicant must indicate in this column that "this unit has no competent department" and affix the official seal of the applicant in this column.

5.2 Opinions of local safety supervision institutions
In accordance with the relevant provisions of the implementation of licensing work, the license that belongs to the General Administration of Quality Supervision, Inspection and Quarantine for examination and approval needs to be signed by the safety supervision organization of the provincial quality and technical supervision department before the special equipment safety supervision organization of the General Administration of Quality Supervision, Inspection and Quarantine formally accepts it; The license approved by the provincial quality and technical supervision department shall be formally accepted only after the safety supervision institution of the municipal quality and technical supervision department signs the opinions.
