Legal analysis: 1. Have a physical examination before you apply for a health certificate. 1, 2 ID cards and one-inch color photos of individuals bareheaded, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for health certificate), fill in the basic information such as name and gender, and so on. Second, if the physical examination results meet the conditions for issuing health certificates, health certificates will be issued after passing the training of health laws and regulations organized by health supervision institutions.
Legal basis: Article 14 of the Administrative Measures for Occupational Health Examination. During the occupational health examination, the employer shall truthfully provide the following relevant materials required for the occupational health examination, and bear the inspection expenses:
(a) the basic situation of the employer;
(two) the types of occupational hazards in the workplace and the list of contact personnel, positions (or jobs) and contact time;
(3) Relevant information such as regular detection of occupational hazard factors in the workplace.
