First, the information to be submitted for license and first registration.
Persons who apply for a nurse's practice certificate and apply for the first nurse's practice registration must provide the following information to the registration authority:
1, nurse's practice certificate and application form for first registration (original);
2, early nursing professional qualification examination (undergraduate) grade certificate (copy of the original inspection);
3. Diploma of nursing (including midwifery) recognized by the state (original copy);
4, the applicant's identity certificate (copy of the original inspection);
5. A valid certificate of the position of nurse to be employed by the medical institution (excluding the position of nurse assistant or assistant nurse and paramedic);
6, the effective proof of clinical practice of medical units and business technology assessment certificate;
7. The applicant's health examination form within 6 months issued by the secondary hospital and maternal and child health hospital where the proposed employer is located (original);
8. Two recent one-inch bareheaded half-length color photos;
9, the personnel file has been in the medical institutions in our province, the undergraduate nursing graduates, the employment notice issued by the personnel department;
10. Those who have passed the national junior (bachelor) nurse qualification examination or have not registered within two years after obtaining the nurse practice qualification shall also submit the certificate that they have been trained in the hospital designated by the health administrative department in charge of registration for 3-6 months and passed the examination when applying for registration.
The only person who does not intend to practice is the Nurse's Practice Certificate, so it is not necessary to provide items 5, 6 and 7 in the above information.
