The third step is to get the certificate. On the day of appointment, bring the Notice of Approval of Establishment Registration and the original ID card of the manager, and go to the Industrial and Commercial Bureau to get the original and copy of the business license.
It is necessary to have the qualification of an independent legal person, the teaching venue, facilities and equipment for carrying out the training activities of health manager qualification certification, and the personnel with relevant health manager qualifications. If it is necessary to apply for a business license or administrative license, the medical institution shall apply for an administrative license. Medical institutions submit application materials to the health window of the government affairs center, and the Health and Health Bureau organizes professionals to conduct on-site evaluation of the applicant institutions.
Health management companies need qualifications to register.
1. Copy of ID card of legal person and partner;
2. The amount of registered capital and the contribution ratio of each shareholder;
3. The company name is more than five characters;
4. The main business scope of the company;
5. One or more shareholders of age 18;
6. All shareholders must produce a valid ID card or a copy of China passport;
7. The registered capital is not less than the statutory capital (according to various industries);
8. Registered address.
Legal basis: Article 12 of the Company Law of People's Republic of China (PRC). The business scope of the company is stipulated in the articles of association and registered according to law. A company may amend its articles of association and change its business scope, but it shall register the change. Projects that are required to be approved by laws and administrative regulations in the company's business scope shall be approved according to law.
Article 9 of the Measures for the Administration of Registration of Individual Industrial and Commercial Households refers to the industry category to which individual industrial and commercial households belong to carry out business activities.
The registration authority shall register the business scope of individual industrial and commercial households according to the application of the applicant and referring to the classification standard in the National Economic Industry Classification.
