Please download the Application Form for Appointment of Occupational Health Examination and the Entrustment Agreement for Occupational Health Examination. Please fill in the Application Form for Appointment of Occupational Health Examination as required.
2, physical examination schedule
According to the time sequence of submitting the Application Form for Appointment of Occupational Health Examination, contact the unit contact person in the Application Form for Appointment of Occupational Health Examination by telephone to communicate and arrange the specific medical examination time, and the medical examination appointment was successful.
Step 3 have a physical examination
The employing unit shall organize workers to go to the occupational disease clinic for occupational health examination on time on the determined date. Please bring two copies of the paper version of the Application Form for Appointment of Occupational Health Examination and the Entrustment Agreement for Occupational Health Examination with official seals during the physical examination.
4, medical expenses and reports
Payment form of medical examination fee: company bank transfer or check (see Note F for specific account information).
Issue an occupational health examination evaluation report within 30 working days after the physical examination of the employing unit. Notify the employer's contact person to collect the report.
