I. Conditions for application and review
(1) The applicant meets the relevant requirements of the Notice on Printing and Distributing the Qualifications of Directors (Medicine, Medicine, Nursing and Technology) in Jiangsu Province (Su Zhiti [2065 438+07] 12).

Three, professional practice ability assessment requirements

Clinical treatment, medical records, diagnosis and treatment plans, epidemiological reports, pathological reports, research and development of drugs and vaccines, cases of psychotherapy and consultation, work summary, and deeds such as commendation, merit and reward obtained in the frontline work of epidemic prevention and control can all be used as evaluation materials.
4. COVID-19 epidemic prevention and control front-line medical staff related policy tilt is limited to one-time enjoyment when promoted to senior titles.
(II) Health professional and technical personnel who change jobs (refers to those who change jobs between five types of jobs: medicine, medicine, nursing, technology and health management) must change jobs for 1 year before they can apply for the qualification of the same level in their current posts. The specific procedures are: the applicant's online declaration and submission of materials, the unit's publicity, the confirmation and review by the unit or the competent department, and the level-by-level review by the health department and the human society department.
(1) Personal declaration (September 6th to September 22nd)
The applicants log in to Jiangsu Health Talent Network (www.jswsrc.com.cn) "Health Advanced Professional and Technical Qualification Declaration Entrance" or Jiangsu Talent Information Port Professional Title Declaration and Evaluation Service Platform (/zc/) to enter the declaration system. According to the system prompt, register and declare online, and upload the corresponding materials as required. After 24:00 on September 22nd, the declaration system will be closed. In principle, overdue supplementary reports, information modification and material supplement will no longer be accepted. If it is really necessary to postpone the supplementary report due to epidemic prevention and control, the Municipal Health and Health Commission shall submit a written application to the Provincial Health and Health Commission.
1. The applicant shall scan or take photos of the original application materials according to the requirements of this notice and the Instructions for Applicants to Submit Materials (see Jiangsu Health Talent Network for details) and the system prompts, and make them into PDF files for classification and uploading. The uploaded materials can be previewed online, and the applicant is responsible for the completeness, accuracy and clarity of the information filled in and the uploaded attachment materials.
2. After the online application and uploading materials are completed, the applicant will export and print the Application Form in triplicate, and bring relevant original materials to the personnel department of the unit for review and seal. Papers, medical records, special reports, business reports, nursing and community health professional and technical work completion tables and attached materials are reviewed by the unit, and need not be provided for on-site confirmation.
3. The materials submitted by the applicant must be true, reliable and traceable, and the Provincial Health and Wellness Committee will conduct spot checks on the original data sources and medical records of the submitted papers. Anyone who forges academic qualifications, qualification certificates and years of service, provides false results and papers, plagiarizes other people's works and academic achievements, once verified, will be disqualified from reporting in the current year.

After the applicant completes the online declaration, it shall be publicized by the personnel department of the unit where it belongs for 7 days, and the relevant units reflected in the publicity shall be checked in time and handled according to relevant regulations.
(3) Confirmation audit (September 7th to September 23rd)
The applicant of the unit below the county level shall bring the "Declaration Form" and the original relevant materials within the specified time, and go to the county (city, district) Health and Wellness Committee for confirmation. Municipal and above units reporting personnel to the unit or the competent department to apply for confirmation. If it is not confirmed within the time limit, it shall be deemed as giving up the declaration.
The staff should check whether the materials submitted online are true, accurate and clear one by one against the original, confirm the declaration information, and sign the corresponding columns in the declaration form. It is reviewed by the human and social departments and stamped on the "Declaration Form". If the application conditions do not meet the requirements or the materials do not meet the requirements, it will not be passed.
The applicant shall provide the following original materials when confirming:

(4) Review by district (deadline 10, 16)
The application materials of each district and city shall be reviewed by the Municipal Health and Health Commission and the People's Social Security Bureau, and stamped on the application form. The pilot provinces and counties (cities) are still submitted according to the original channels.
(5) Provincial Final Appeal
The Provincial Health and Wellness Committee and the Provincial People's Social Welfare Department will conduct a final review of the application materials. After 5 working days, the approved personnel will be publicized on Jiangsu Health Talents Online and submitted to the Higher Jury for review.
