Matters needing attention in orthopedic practice

In clinical teaching, the teaching method remains unchanged, students sound sleepy, and the teaching effect can not be achieved at all. Our hospital is a comprehensive teaching hospital with many interns and different educational levels. In addition, the theory and operation technology of orthopedics are relatively professional, and the practice time of nursing students is short (usually 4 weeks), and the teaching frequency is fast. According to the above characteristics, and according to the educational level, a teaching plan is formulated that conforms to the characteristics of undergraduate course: secondary school students are required to master various basic nursing skills and theories within 4 weeks; In addition to mastering nursing skills and professional theories, college students also need to master the working methods of holistic nursing procedures, communicate well with patients and their families, and implement health education; Undergraduate students should not only meet the requirements of internship, but also pay attention to students' teaching, management ability and scientific research consciousness. Therefore, we divide the practice into three stages, adopt different teaching methods according to the specific conditions of each stage, set weekly knowledge goals and skills goals, set specific contents and operations, and effectively teach nursing students to better complete their studies.

Week 1 week: familiarization stage

Take the form of collective teaching: usually the head teacher is in charge.

1. On the first day of entering the department, the environment, layout and facilities of orthopedics were introduced, and the responsibilities of personnel at all levels were divided to eliminate strangeness. Then spend 2 ~ 3 days studying professional ethics and various rules and regulations, so that students can deeply understand the importance of standardized operation in orthopedics, explain the names, uses, maintenance, uses and methods of commonly used instruments, the precautions in nursing operation and demonstrate the operation, and patiently guide nursing students to practice until there is no doubt after the explanation;

2. The theoretical teaching content is basic nursing knowledge of orthopedics. Knowledge Objective: To understand the layout of orthopedics, the internal facilities of orthopedics and the bed area of wards, and to focus on mastering the operational skills of life nursing and basic nursing. For example, in order to change bed sheets for bedridden patients, it is more important to understand the comfort of patients and the special posture required by orthopedic patients, in addition to emphasizing the cleanliness of operation time and bed angle. Pay attention to people-oriented, consider patients' feelings, skillfully use language communication skills and explain terms according to nursing procedures and methods. Familiar with inspection system, specimen management system and aseptic technique operation principle. Familiar with disinfection and sterilization methods and precautions of various bone traction instruments.

Week 2-3: Practical stage

This stage mainly adopts one-on-one teaching methods:

1. With the development of "patient-centered" holistic nursing work, our department has selected nurses with rich experience, high seniority and good comprehensive quality to teach. Establish a "student-centered" teaching concept, teach students in accordance with their aptitude, and teach students in accordance with their aptitude. Taking 2 ~ 3 typical orthopedic cases as clinical nursing teaching links, the basic nursing technology operation, professional theoretical knowledge, functional exercise and health education were integrated into case nursing. Every teacher is required to enhance teaching awareness, be patient and meticulous, let go and look around, and pay attention to words and deeds. In the teaching process, help nursing students learn to learn and give full play to their initiative. At the same time, combined with the clinical practice of orthopedics, the training content is put forward to help nursing students establish a new learning thinking mode.

2. Nursing students must perform various operations under the guidance of teachers. Master the nursing knowledge before and after orthopedic surgery, as well as the nursing measures for patients with plaster external fixation and traction, the rehabilitation knowledge of hip and knee replacement, and the knowledge of orthopedic surgery. Demonstrate the method of turning over for patients with skull traction and spinal fracture on the spot, and require nursing students to participate in functional exercise for paralyzed patients and health education for orthopedic diseases.

3. Nursing students take part in day shift and night shift work. Accelerate the understanding and transformation of the role of nurses and the in-depth understanding of nursing work.

4. At this stage, teachers should constantly enrich their theoretical knowledge and work experience. At the same time, teachers should pay attention to cultivating students' interest in learning and adopt diversified teaching methods such as model teaching and multimedia teaching to make boring theoretical knowledge vivid and vivid. In addition, the head nurse instructors organize/kloc-0 nursing lectures and 2 nursing rounds every month, covering the observation of common orthopedic diseases, nursing rescue measures, experiences and lessons. Encourage nursing students to ask questions constantly during the internship and learn with questions. In addition to completing the centralized teaching content at this stage according to the teaching plan, the teaching content can be increased at any time according to the actual situation.

Week 4: Final evaluation stage

Take the form of examination and mutual discussion: an important link in the quality of clinical teaching is strict examination. Schedule the exam for the fourth week. The examination content is divided into theoretical examination and operational examination, covering all the contents required by the internship plan. Head nurses and class teachers take part in invigilation and questionnaire surveys, record their achievements, fill in the internship appraisal seriously and responsibly, summarize the internship situation, praise the advanced and encourage the backward. Teachers discuss with students, solicit nursing students' opinions and requirements on teaching, and constantly improve teaching methods. Teachers dare to face students' evaluation, and at the same time, as friends, let students point out the problems existing in teaching, remind students of the problems existing in practice, help students find out the shortcomings and put forward targeted opinions.

Orthopedic nursing is a highly operational specialty. Through clinical practice, the combination of theory and practice has improved the learning interest of nursing students, increased their learning enthusiasm and cultivated their practical ability. By adopting the above-mentioned phased learning methods and measures, students can be familiar with the working environment of orthopedics in a short time, master strict operation techniques, combine centralized training with individual guidance, and improve the quality of clinical teaching.
