2. This registration form is included in the student file. Please fill it out carefully. Alteration during filling is strictly prohibited. If correction fluid is used, this form will be invalid.
3. The items listed in the table must be filled in truthfully, and no spaces are allowed. If the information is unclear and cannot be filled in, it should be written as "unclear" and "unknown" and their reasons. If you don't have this information, you should also write "None".
4. The full name of "school name", "department" and "major" on the cover of the form must be filled in.
5. The "Name" column on the front cover and home page of the form must be filled in the same way as the ID card, household registration book and student ID card. If there is a former name, fill it in truthfully, and if there is no former name, fill in "None".
6. "Now detailed home address" should fill in the parents' home address, and married people can fill in their own home address instead of the dormitory address of the school. "Postal Code" and "Home Phone Number" should be consistent with the "Existing Home Address" filled in.
7. "I am in good health" is generally filled with no disease, or "healthy" or "good". If there is any disease, it should be truthfully filled in.
8. In the column of "Marriage, the other party's name, political outlook, current location and position", the unmarried person should fill in "None", and the married person should fill in all the contents of the spouse's name, political outlook, current unit and position, and fill in clearly.
