1. When filling in, all pens or signature pens (black) should be used. Do not use ballpoint pens or pencils, and the handwriting should be clear.

2. This registration form is included in the student file. Please fill it out carefully. Alteration during filling is strictly prohibited. If correction fluid is used, this form will be invalid.
