Enter the primary and secondary school teacher qualification examination network, click the home page to enter 202 1 details of the registration notice for the secondary school teacher qualification examination (written examination).
Pull to the bottom attachment, find the teacher qualification certificate to apply for the provincial epidemic prevention and control notice, and click on the "Announcement of written epidemic prevention and control for primary and secondary school teachers" to enter the details.
Pull the "Candidate Health Self-Commitment Letter" attached to the teacher qualification certificate at the bottom of the notice, and click Download and Print.
Materials to be submitted for taking the teaching examination:
In the teacher qualification examination, candidates must provide paper materials such as self-commitment letter of health status, negative certificate of nucleic acid test, and commitment letter of exam integrity. And you also need to show your health code, travel itinerary, ID card and teacher qualification certificate admission ticket.

Proof materials that candidates must prepare before entering the test center and examination room of teacher qualification certificate. Candidates must present their Health Code (including Communication Big Data Travel Card), valid resident ID card and teacher qualification certificate admission ticket every time they enter the test center.
Every time a candidate enters the test center of teacher qualification certificate, he/she must submit his/her signed "Self-Commitment Letter on Candidates' Health Status" (attachment 1) and twice negative nucleic acid test within 7 days and 48 hours.
Negative nucleic acid test proves that all four forms are effective. See Annex 2 for an example. After returning to school this semester, students who are in the closed-loop management of epidemic prevention and control and have not left school to take the teacher qualification examination in our school do not need to provide negative proof of nucleic acid test within 7 days and 48 hours.
