The family population can be empty first, fill in the third part of the form and then come back to fill in this item. The number is the number of family members+1 (plus 1 is actually yourself).
In the part of family members, fill in the name of family members, their relationship with themselves, age, ID number, work unit or school, position or grade, monthly income, etc. If there is more than one family member, you can attach another page to continue to fill in.
In the part of family economic situation, fill in the total income, total expenditure, average monthly income and expenditure of the family. If there are special difficulties or other situations that need to be explained, please explain them in the remarks column.
In the applicant's commitment section, fill in the application date and sign at the applicant's signature.
The opinions of the school shall be filled in by the relevant person in charge of the school and stamped with the official seal of the school.
