First, medical and health institutions that carry out occupational health examination in our province must be approved by the Provincial Health Department.
Two, the Health Supervision Bureau of the Provincial Health Department is responsible for the acceptance, data review and organization of experts to conduct on-site review, review opinions, approval, certification and other work.
Three, the applicant must have the institutions, personnel, equipment, quality management system and other related conditions stipulated in the "Qualification Examination Conditions for Occupational Health Inspection Institutions in Hebei Province" (see Annex).
Four, the applicant directly to the Provincial Health Department of Health Supervision Bureau to apply, and submit the following information:
(1) Application Form for Occupational Health Inspection Institution in Hebei Province (in duplicate);
(2) Legal person qualification certification materials (copy)
(3) Practice License of Medical Institution (copy);
(4) The applied occupational health examination items (filled in according to the hazard factors and operations listed in "Occupational Health Examination and Cycle" in the Administrative Measures for Occupational Health Surveillance);
(five) technical personnel (foreign personnel must submit proof materials), equipment and other materials that are suitable for the occupational health examination project;
(6) Occupational health examination quality management documents and relevant rules and regulations.
Five, the examination and approval procedures for the qualification of occupational health inspection institutions shall be handled in accordance with the "Audit Procedures for Vocational and Technical Service Institutions in Hebei Province".
Six, occupational health inspection institutions should implement the relevant provisions of the "Occupational Disease Prevention Law" and "Occupational Health Monitoring Management Measures", and earnestly fulfill their legal obligations. Anyone who engages in occupational health examination without approval, engages in health examination beyond the scope, fails to perform legal duties, issues false certificates and other illegal acts will be given administrative punishment according to law once verified.
