The crew must go to a hospital recognized by the Maritime Safety Administration for a physical examination when changing their certificates. There must be a 2-inch bareheaded color photo during the physical examination, which must be consistent with the photo you gave to the Maritime Safety Administration when changing your certificates.
The health certificate is only a certificate for your physical examination on board, and it has nothing to do with your replacement.
