Second, the occupational health records management requirements:

1. The employing unit shall establish occupational health files in accordance with the relevant laws and regulations on occupational disease prevention and control and the requirements of this notice, which shall be comprehensively managed by the occupational health management department, formulate management systems such as borrowing and registration, and designate special personnel to manage them; If the employing unit is a head office or a group company, the occupational health records of the headquarters of the company may be simplified appropriately, but its subordinate employing units must establish occupational health records according to the requirements of this notice.

2. The occupational health record consists of a summary document and twelve sub-documents. Summary documents and twelve sub-documents shall be classified and stored in a unified file box, and the file box shall be marked with the file name.

3, "Occupational Health Archives" is an important basis for occupational health supervision and management and supervision and law enforcement, should be permanently preserved and properly kept. When the management personnel change, the handover work should be done well.

4, "Occupational Health Records" involves the personal privacy of employees and confidential information of the unit, which should be kept confidential.

5. If the employer involves more projects and personnel, you can refer to the sample table to add tables to supplement.

6. The style of occupational health files can be appropriately adjusted according to the actual work, but the main contents involved cannot be lacking, and the settings of the general file and twelve sub-files cannot be reduced.

7. In case of division, merger, dissolution or bankruptcy of the employing unit, the occupational health records shall be handed over in accordance with the relevant provisions of the national archives management and properly kept.

8. The employing units that have established occupational health records before the issuance of this notice shall re-improve them according to the requirements of this notice and file them by classification.

Legal basis:

Article 20 of the Law of People's Republic of China (PRC) on the Prevention and Control of Occupational Diseases, the employing unit shall take the following measures for the prevention and control of occupational diseases:

(a) to set up or designate an occupational health management institution or organization, with full-time or part-time occupational health management personnel to be responsible for the prevention and control of occupational diseases in the unit;

(2) Formulating occupational disease prevention and control plans and implementation plans;

(three) to establish and improve the occupational health management system and operating procedures;

(four) to establish and improve the occupational health files and workers' health monitoring files;

(five) to establish and improve the monitoring and evaluation system of occupational hazard factors in the workplace;

(six) to establish and improve the emergency rescue plan for occupational disease hazards.
