Article 2 These Measures are interest-oriented policies that give necessary financial rewards and subsidies to special family planning families (families whose only child is disabled or dead and has not given birth again) before the adjustment of the general two-child policy within the administrative area of this Municipality, and couples of rural one-child families whose women have implemented measures of going to Sheung Wan or withdrawing from childbearing age. Including family planning special family one-time assistance and rural one-child family husband and wife reward (hereinafter referred to as "implementation of environmental measures or withdrawal from childbearing age reward").
Article 3 The objects of reward and assistance are family planning special families registered in this Municipality and rural one-child families whose women have implemented the measures of going to the ring or withdrawn from childbearing age. The object of reward and assistance shall meet the following conditions:
(1) Family planning special families.
1. At least one of the husband and wife is registered in this city;
2. The woman has reached the age of 45;
3. The universal two-child policy conforms to the policy before adjustment and only gives birth to one child for life (including legal adoption);
4. No surviving children or only children are identified as disabled according to law (disability reaches level 3 or above);
5. If it belongs to a single-parent family, the original spouse who has an only child with it should be at least 45 years old; If the woman belongs to a single-parent family, she must be at least 45 years old.
(two) couples in rural one-child families who have implemented the measures of going to Sheung Wan or withdrawing from childbearing age.
1. Rural household registration in this city, in which the man is under 60 years old and the woman is under 55 years old;
2. Before the adjustment of the universal two-child policy, the woman has implemented the Sheung Wan measure or withdrawn from the childbearing age;
3. Only have one child (including legal adoption).
The two types of reward and assistance objects stipulated in this article include those who meet the prescribed conditions, such as widowhood, divorce and remarriage; The so-called "withdrawal from childbearing age" usually refers to a period of time after the woman reaches the age of 49.
Article 4 Standards of reward and assistance.
(a) in accordance with the provisions of these measures, the one-time relief object of special family planning, issued by the local government in accordance with the standard of per household 10000 yuan.
(two) in accordance with the provisions of these measures, the woman has implemented Sheung Wan measures or withdrawn from the rural one-child families of couples of childbearing age, and the local government of the household registration will issue bonuses according to the standard of 50 yuan per person per month. From the next month of eligible applications, it will be distributed until men reach the age of 60 and women reach the age of 55.
Article 5 The reward and assistance funds shall be borne by the finance, which shall be shared by the city, Huicheng District, Huiyang District, Daya Bay Development Zone and Zhong Kai High-tech Zone in a ratio of 4:6, and Huidong County, Boluo County and Longmen County shall bear their own expenses.
Article 6 Individuals who meet the conditions of reward and assistance in these Measures may apply to the village (neighborhood) committee where the household registration is located, and fill in the Application Form for Huizhou City to Implement Sheung Wan Measures or Withdraw from the Reward and Assistance of Childbearing Age or the Application Form for One-time Assistance to Family Planning Special Families in Huizhou City (hereinafter referred to as the Application Form).
(a) in rural one-child families, couples of childbearing age whose women have implemented the Sheung Wan measure or withdrawn from it shall provide the following materials when applying:
1. Original and photocopy of my resident ID card, household registration book and marriage certificate;
2. The original and photocopy of the honor certificate of the one-child parents or the one-child preferential card, the legal adoption certificate or the adoption notarial certificate;
3. Two one-inch bareheaded recent photos;
4. Fill in the application form (in duplicate) and sign the application commitment letter.
(two) family planning special family one-time relief object, need to provide the following materials to apply:
1. The original and photocopy of the resident ID card and household registration book of both husband and wife.
2. The original and photocopy of the marriage certificate or divorce certificate of both husband and wife. Immigrants should cooperate to verify their family planning matters such as marriage and childbirth. The object of divorce or remarriage shall provide the original and photocopy of the divorce judgment or divorce agreement.
3. The original and photocopy of the one-child parents' honor certificate or one-child preferential card, legal adoption certificate or adoption notarial certificate.
4. Only-child disabled families need to provide only-child disabled cards (level 3 or above); Families whose only child died must provide cremation certificates or death certificates issued by medical institutions or death cancellation records of household registration books.
5. Two one-inch bareheaded recent photos.
6. Fill in the application form (in duplicate) and sign the application commitment letter.
Seventh reward and assistance object qualification confirmation procedures.
(1) First trial. The village (neighborhood) committee shall, after receiving the applicant's "Application Form" and related materials, complete the preliminary examination within 5 working days according to the confirmation conditions of the object of reward and assistance, and inform the applicant of the materials that need to be supplemented at one time. To meet the conditions, put forward opinions and submit the list of preliminary examination objects to the township (street) health institutions; Do not meet the conditions, should explain the reasons to the applicant; For those who have moved in from outside the province or outside the city, there is no honor certificate for the one-child parents or the one-child preferential card. I think that if they meet the application conditions, the applicant will sign a letter of commitment that meets the conditions, and the village (neighborhood) Committee will hand it over to the township (street) health work agency for verification. At the time of preliminary examination, the list of one-time relief objects for special family planning needs to be posted simultaneously for 5 days and accepted by the masses.
(2) review. Township (street) health institutions shall strictly examine the applicant's application materials and commitments within 5 working days after receiving the list of preliminary examination objects, and summarize the list of objects that have passed the examination to form the Registration Form of Huizhou City's Implementation of Sheung Wan Measures or Withdrawal of Childbearing Age Rewards and the Registration Form of Huizhou City's One-time Relief Objects for Family Planning Special Families (hereinafter referred to as the Registration Form), with audit opinions attached, and submit the application form and registration form together before the 20th of each month. For those who move in from outside the province or outside the city, there is no honor certificate of the one-child parents or the one-child preferential card. The situation of marriage and childbearing should be further verified through the Guangdong Provincial Population Information System or other effective inspection methods. Need to contact the field investigation and verification, the audit can be extended by 10 working days.
(3) confirm. The county (district) health department shall confirm the list of reward and assistance objects submitted by township (street) health institutions, and the confirmation work shall be completed within 5 working days. After confirmation every month, the list of reward and assistance objects, the registration form and the application form will be returned to the township (street) health institutions, step by step to the village (neighborhood) committee for the record, and the list of reward and assistance objects in each township (street) and the required funds will be summarized, and will be reported to the finance department at the same level in five working newspapers. For those who do not meet the requirements after examination, the reasons shall be explained to the applicant through the township (street) health work institution.
(4) publicity. The list of reward and assistance objects confirmed by the audit was publicized to the masses in the village (neighborhood) Committee for 5 days. The masses have objections, the village (neighborhood) committee should report to the township (street) health institutions, and verify the situation within 5 working days, special circumstances can be extended 10 working days. If it is verified that it does not meet the conditions for reward and assistance, the township (street) health agency shall report to the county (district) health department to cancel the qualification for reward and assistance.
(5) inform. Township (street) health institutions to the county (District) health department to confirm the reward and assistance object, inform the reward and assistance object within 5 working days.
Article 8 When an object of reward and assistance applies for assistance such as minimum living security, support for destitute people, marginal families with minimum living security, and families with financial difficulties, the reward and assistance funds shall not be included in family income. Bonuses are distributed by the health department of the county (district) in accordance with the Implementation Plan of Huizhou Finance Bureau on Printing and Distributing Financial Subsidies for Benefiting the People and Benefiting the Farmers (No.[2020]1).
(a) has been implemented in Sheung Wan or withdraw from the payment of childbearing age bonuses measures.
1. Rewards that have implemented the measures of going to Sheung Wan or withdrawing from childbearing age will be paid monthly from the month after the reward object applies;
2. During the bonus payment period, if the reward object is divorced, widowed or remarried and no longer gives birth, the bonus will continue to be paid;

4. As the villagers' committees are transformed into residents' committees and the original rural residents are transformed into urban residents, they can continue to enjoy the implemented Sheung Wan measures or withdraw from the childbearing age within four years after the transformation.
(two) a one-time relief fund for special families with family planning.
1. One-time relief funds for family planning special families shall be paid in the month after the recipient applies.
2. Families who have reached the age of 60 and have not received disability and death relief funds will be reissued with one-time relief funds for family planning special families according to the current standards. If a couple in a family planning special family with the household registration in this Municipality divorced before the death of an only child, and if a couple in a family planning special family with a disabled only child divorced, the party who raised the only child determined by the divorce agreement or judgment shall apply for relief funds; Special family planning couples who divorce after the death of their only child apply for 50% assistance. If one of the spouses dies or is not registered in this city, the party registered in this city shall apply for full assistance.
Article 9 In any of the following circumstances, the bonus application procedures shall not be handled.
(1) Failing to fill in the application form as required;
(2) Failing to provide valid application materials as required;
(three) children born out of wedlock, did not go through the marriage registration procedures;
(4) adopting a child without providing the adoption certificate issued by the civil affairs department or the adoption certificate issued by the notary office;
(five) divorced, remarried families and families over 60 years of age have received a one-time assistance from special family planning in full according to the regulations;
(six) other objects that do not belong to the provisions of these measures.
Tenth has been implemented in Sheung Wan measures or withdraw from receiving bonuses of childbearing age reward object, one of the following circumstances, cancel or terminate its reward qualification.
(a) receiving the implementation of Sheung Wan measures or withdrawing from childbearing age to have children again;
(two) the account moved out of Huizhou City or settled abroad;
(3) voluntarily changing the status of rural residents into urban residents;
(four) the death of the reward object;
(five) providing false documents and materials and the content of the commitment has not been confirmed.
Eleventh village (neighborhood) committee responsibilities.
(a) to assist the township (street) health institutions to do a good job in the publicity of these measures.
(two) to assist the township (street) health institutions to investigate and verify the reward and assistance objects in the registered population within their jurisdiction, inform the parties to declare the eligible objects in time, and assist the applicants to fill in the application form and establish files.
(3) the preliminary examination of the applicant's application form, timely publication of the preliminary list of one-time relief objects for special family planning.
(4) Publicizing the confirmation list of reward and assistance objects;
(five) every six months to assist the township (street) health institutions to check the implementation of Sheung Wan measures or withdraw from the childbearing age reward object fertility. For the reward object that has given birth to a second child, put forward the suggestion of stopping the bonus in time; If the reward and assistance object is suspected of giving birth to a second child, it shall be reported to the township (street) health work institution for verification.
Twelfth township (street) health work responsibilities.
(a) to do a good job in the publicity of these measures;
(two) responsible for the examination and approval of the qualifications of the object of reward and assistance;
(three) classified registration, the establishment of information files, the use of information technology to establish relevant information files for qualified reward and assistance objects, and inform the reward and assistance objects themselves in writing;
(4) Every six months, through the Guangdong Provincial Population Information System, check the fertility status of people who have implemented the measures of going to the ring or withdrawn from childbearing age, report to the county (district) health department in time, and stop issuing bonuses to those who have given birth to a second child.
Thirteenth city and county (District) health department responsibilities.
(a) the municipal health department shall guide, supervise and inspect the county (District) health department and the county (District) health department's reward and assistance work for township (street) health institutions;
(two) the county (District) health department is responsible for the one-time relief object of family planning special families and the confirmation of the implementation of the ring measures or the withdrawal qualification of the reward object of childbearing age;
(3) Before the county (district) health department formulates the reward and assistance plan every year, it shall investigate the number of reward and assistance targets in the next year and formulate the reward and assistance plan, and timely submit it to the finance department at the same level and the municipal health department; The municipal public health department shall summarize the survey results of the county (district), formulate the city's reward and assistance plan, and submit it to the municipal finance department in a timely manner, and cooperate with the finance department to do a good job in budgeting and raising reward and assistance funds.
Article 14 The financial departments of cities and counties (districts) shall be responsible for the guarantee of reward and assistance funds, review the budget of reward and assistance funds provided by the health department, incorporate it into the next fiscal budget, timely allocate funds in accordance with the provisions of centralized payment by the state treasury, strengthen the supervision of budget implementation and post supervision, and implement the final report system in conjunction with the health department at the same level.
Article 15 Responsibility of financial institutions. To provide efficient and high-quality services for cardholders, after receiving the list entrusted by functional departments, timely transfer the reward and assistance funds to the reward and assistance object account, indicate the year, fund type, amount and other details in the payment information and remarks, and feed back the payment data and error information.
Sixteenth county (District), township (street) health departments should set up and publish the reward and assistance work to report complaints telephone, timely accept complaints from the masses.
(a) township (street) health institutions and village (neighborhood) committees refuse to apply for objects that meet the conditions of reward and assistance, and the parties may complain to the health department at a higher level. Can be handled by the health department at a higher level, township (street) health institutions and village (neighborhood) committees shall handle it within 5 days after receiving the notice from the health department at a higher level. If it is not handled on time, the responsibility of relevant personnel shall be investigated according to regulations.
(two) in line with the conditions of incentives and assistance for the applicant to receive incentives and assistance, the village (neighborhood) committee, township (street) health institutions shall not charge any fees to the parties. If it happens, the object of reward and assistance can complain to the health department at the next higher level. Once verified, the relevant personnel shall be investigated for responsibility.
Seventeenth reward and assistance funds must be earmarked, and no unit or individual may illegally occupy them. If it happens, it will be dealt with according to law.
The management and distribution of reward and assistance funds shall be subject to the supervision of discipline inspection and supervision organs and audit departments at all levels according to law.
Article 18 These Measures shall come into force as of June 65438+1 October1day, 2023, and shall be valid for 5 years.
