Three. Long-term work arrangement:

1, health file. Continue to establish and improve information files, update files in time, and do a good job of confidentiality.

2. Chronic disease management. Conduct at least 4 face-to-face follow-up visits to high-risk groups of chronic diseases such as hypertension and diabetes, provide regular consultation services and medication guidance, and input them electronically in time. Make use of follow-up to publicize the knowledge of disease prevention and do a good job in data collection and information reporting. People over 35 have their blood pressure measured in the first clinic. At the same time, strengthen the screening of key populations and register and manage patients with hypertension and diabetes.

3. Health education. Real and meaningful. 7. Prevention and treatment of infectious diseases.

8. Child health care. 9. Maternal health care. Provide basic health care services for pregnant women within their jurisdiction free of charge, standardize maternal health care, and do a good job in early pregnancy.
