A. Bring your ID card and two one-inch bareheaded color photos, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for health certificate), and fill in the basic information such as name and gender.

If the physical examination results meet the conditions for issuing health certificates, a health certificate shall be issued after passing the training on health laws and regulations organized by health supervision institutions. If the physical examination results do not meet the conditions for issuing a health certificate, a health certificate shall not be issued.
Generally speaking, the hospital that provides physical examination will issue it after the examination, or go to the CDC for physical examination and treatment.

Article 5 The competent department of public places shall establish a health management system, equip full-time or part-time health management personnel, regularly check the health status of their business units (including individual operators, the same below), and provide necessary conditions.
Article 6 A business unit shall be responsible for the hygiene management of the public places it operates, establish a hygiene responsibility system, and conduct hygiene knowledge training and assessment for employees.
