1. What materials do I need to bring to apply for a health certificate?
Bring two two-inch registration photos, ID card and counter registration form.
Second, the health certificate processing process is as follows:
1. Payment: show your ID card, pay the fee and receive the invoice. Take photos, have a physical examination, and get a certificate with an invoice. Please keep the receipt properly.
2. Registration for taking photos: Please show your ID card and invoice to register for taking photos;
3.x-ray: If you have a medical history or existing discomfort, please inform the doctor;
4, intestinal sampling: pregnant women please show the pregnancy certificate to the doctor, do not see through;
5, internal medicine and skin examination: please report to the doctor for patients with anal diseases;
6. Blood collection: Blood collection is the last step of physical examination. You can't draw blood without doing any of the steps 2 to 5. After blood collection, the blood collection point should be compressed for 3-5 minutes. If you feel dizzy, please explain to the nurse that the blood will rest for 5 minutes after blood collection, and you can leave without abnormality;
7. Get the results: Please get the health certificate or physical examination report with your invoice and ID card after four working days. Adult floating women of childbearing age whose registered permanent residence is outside the urban area of Wuhan must also show their marriage and childbearing certificates.
To sum up, there are two kinds of health certificates: one is general health certificate, and the other is food health certificate, which is needed by people in the catering industry. Physical examination place: local disease prevention and control center or medical and health institution approved by health administrative department to undertake preventive health examination. If this work is carried out without approval, the health supervision institution will not accept the physical examination form issued by it and will not issue a health certificate.

Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.
Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
