Contents of health certificate:
1. Name, gender, date of birth, nationality, department on board, signature and photo of the holder.
2. Certificate number.
3. Date of issuance and expiry date of validity.
4. Applicable provisions of relevant international conventions.

6. Name of authorized institution.
7. Name of issuing institution and signature of chief physician.
