What documents do seafarers need to apply for a one-year health certificate?

Health certificate refers to the written occupational health certificate issued by the maritime administrative agency according to the Regulations on the Administration of Seafarers in People's Republic of China (PRC) and the international treaties that China has acceded to or concluded, indicating that seafarers' physical condition can effectively perform their duties.

Contents of health certificate:

1. Name, gender, date of birth, nationality, department on board, signature and photo of the holder.

2. Certificate number.

3. Date of issuance and expiry date of validity.

4. Applicable provisions of relevant international conventions.
