Rules and application process of health code conversion
First, the code conversion rules.
1. After 14 days of centralized isolation medical observation, the confirmed cases were changed from red team to yellow code with nucleic acid detection certificate, and after 14 days of home health monitoring, the yellow code was changed to green code with negative nucleic acid detection certificate.
2. After the asymptomatic infected persons were released from centralized isolation medical observation, they changed from red team to yellow code with nucleic acid detection certificate, and continued to carry out family health monitoring 14 days later, the negative certificate with nucleic acid detection changed from yellow code to green code.
3. Close contacts of confirmed cases and asymptomatic infected persons will be changed from red team to yellow code after the expiration of 14 days of centralized isolation medical observation, and from yellow code to green code after 7 days of home health monitoring.
4. Close contacts between the confirmed cases and asymptomatic infected persons, after 7 days of centralized isolation medical observation, will change from red team to green code, and the nucleic acid test will be negative.
5. After 14 days of centralized isolation medical observation, the negative certificate of nucleic acid test changed from red team to yellow code, and after 7 days of home health monitoring; With the negative proof of nucleic acid detection, the yellow code is changed to the green code.
6. The space-time companion of the confirmed case or asymptomatic infected person will be changed from red code to yellow code after the expiration of the negative certificate of nucleic acid detection in centralized isolation 14 days; After the 7-day home health monitoring expires, the yellow code will be changed into the green code with negative nucleic acid test certificate.
7. Residents in high-risk areas, after the expiration of centralized isolation medical observation 14 days, changed from red team to yellow code of negative certificate of nucleic acid detection; After the 7-day home health monitoring expires, the yellow code will be changed into the green code with negative nucleic acid test certificate.
8. Residents in medium-risk areas will be changed from yellow code to green code after the 7-day family health monitoring expires and the nucleic acid test proves negative.
9. Residents in special areas whose last exposure time was within 14 days changed from red team to yellow code after implementing centralized isolation medicine 14 days, and continued to implement home health monitoring for 7 days after the expiration, and changed from yellow code to green code after implementing negative proof of nucleic acid testing. If the last exposure time is more than 14 days and less than 2 1 day, the 7-day home health monitoring will expire, and the yellow code will be changed to the green code with the negative proof of nucleic acid detection. If the last exposure time exceeds 2 1 day, the yellow code will be changed to green code according to the negative results of two nucleic acid tests within 3 days.
Second, the transcoding process
Those who have been assigned red team and yellow codes have been released from centralized isolation and home health monitoring, and meet the requirements for red team and yellow codes to be converted into green codes. Those who have not been converted into green codes for more than 48 hours, or other new personnel who have objections to the allocation of red team and yellow codes, shall apply for transcoding by their family members or friends in their townships. When applying, I should submit my health code conversion application form, notice of canceling concentration or home isolation, and negative proof of nucleic acid test.
Villages and towns are responsible for accepting transcoding applications from code red and code yellow personnel and reviewing materials. The county epidemic prevention and control headquarters shall designate a special person to summarize and verify the information of transcoding personnel and upload it to the municipal epidemic prevention and control headquarters in time. The big data bureau of the municipal government uploads the information of transcoding personnel through the provincial platform and applies for transcoding.
