When the health certificate expires, it needs to be reissued, but it needs to be re-examined as before, and the payment is the same as before (whether the price increase depends on the local CDC). If the physical examination items are normal, you can get a health certificate, which is also valid for one year.
The health certificate cannot be changed after it expires. You need to reapply. The basis is as follows:

Employees must have a physical examination every year before or during their posts, and they need to carry two one-inch photos with them. Only after passing the physical examination and receiving health knowledge training can you get a health certificate. Regional centers for disease control and prevention (or health and epidemic prevention stations).
Article 45 of the Food Safety Law of People's Republic of China (PRC) shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.
Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.

Article 2 Preventive health examination refers to the health examination before, during and during the post of employees engaged in the production and operation of food and drinking water, those directly engaged in the production of cosmetics, those who directly serve customers in public places, hazardous workers, radiation workers and school students in accordance with relevant national health laws and regulations.
Article 3 The preventive health examination shall be managed by the health administrative department of the provincial government in a unified way, and the health administrative departments at all levels shall be responsible for the preventive health examination within their respective jurisdictions.
