Where is the little red book of Anshan health certificate?

Issued by Anshan Health and Epidemic Prevention Station. The work of handling health certificates shall be carried out by Anshan Municipal Center for Disease Control and Prevention or medical and health institutions approved by the administrative department of health to undertake preventive health examination. If this work is carried out without approval, the health supervision institution will not accept the issuance of health certificates.
