Bring the original and photocopy of ID card and one-inch bareheaded photo to the local CDC for physical examination. Before handling the health certificate, you should have a physical examination, bring your ID card and two one-inch color photos, pay the medical examination fee at the toll booth, get the medical examination form, paste the photos (the other one is for handling the health certificate), fill in the basic information such as name and gender, draw blood from the laboratory, conduct stool swab tests, conduct routine medical and surgical examinations, and do X-ray chest X-rays to complete all physical examination items.
