(1) Original and photocopy of the child's birth certificate.
(2) The original and photocopy of the household registration book.
(3) The original and photocopy of the real estate license (required by the kindergarten).
(4) Children's exemption certificate (check and stamp at the children's inoculation place in advance).
(5) Before children officially enter the park, they must go to the maternal and child health care station in their district for physical examination. Only after passing the medical examination can you officially enter the park.
Some enterprises and institutions run parks to identify whether they are employees' children, and also require work permits and certificates for purchasing social security in recent months.
When you sign up, you usually bring your children.
What needs special emphasis here is the baby's physical examination report. Physical examination is generally carried out in the district maternal and child health station (the kindergarten will inform the physical examination location after registration). The main examination items include: height and weight measurement, nutritional status evaluation, vision and hearing screening, etc. Through physical examination, parents and kindergartens can grasp the healthy and nutritional status and physical development of children before entering the park in time, and issue the Physical Examination Form for Children Entering the Park to those who pass the physical examination.
Note: Physical examination in the park not only allows kindergarten teachers to know the health status of their children, but also allows parents to fully understand the health status of their children. Generally speaking, except in special circumstances, the results of physical examination will not affect children's admission to the park.
