Get a health card:
1. With my original ID card of the second or third generation, I can handle it at the networking institution designated by the district and county health bureau where the resident health card is issued.
2. Applicants under the age of 16 must have a legal guardian with an original valid ID card, a photo of the applicant, a household registration book or other valid relationship certificates. For the specific application process, please log on to the websites of district and county health bureaus.
3. After the personal information of the declared residents is finally audited by the registration management center of the Health Bureau and the financial department, the registration management center of the Health Bureau will make unified arrangements for making the residents' health cards.
4. After the residents' health cards are made, they will be distributed by the Postal Savings Bank or collected at bank outlets.
Resident health card is a CPU card that integrates social security card, new rural cooperative medical system card and medical institution card. Residents' health card is the basic carrier of personal information proposed by the state to realize cross-system, cross-institution and cross-regional interconnection and information sharing of medical and health services. The resident health card records all medical information from birth to death.
The materials required to obtain a health certificate are as follows:
1, original and photocopy of the second-generation ID card;
2. Two recent photos of 1 inch;
3. Students should also bring their registered student ID cards;
4. Original and photocopy of marriage and childbearing certificate of floating population in other provinces.
Certification process:
1. Have a physical examination before applying for a health certificate;
2. Bring your ID card and two one-inch color photos without a hat, receive the medical examination form, pay the medical examination fee at the toll booth, paste the photos (the other one is for health certificate), and fill in the basic information such as name and gender;
3. Laboratory blood collection and stool anal swab test;
4, internal and surgical routine physical examination;
5, X-ray chest X-ray;
6. Complete all physical examination items;
7. If the medical examination results meet the conditions for issuing health certificates, if the medical examination results do not meet the conditions for issuing health certificates, no health certificates will be issued. After passing the training of health laws and regulations organized by health supervision institutions, I will go to the health supervision office of the health bureau under my jurisdiction to apply for a health certificate with my bareheaded color photo.
To sum up, the resident health card is mainly used for resident identification, basic health information storage, cross-regional and inter-institutional medical treatment, expense settlement and financial services in medical and health service activities.
