In order to further enhance the prevention and control capabilities of medical institutions in COVID-19, consolidate the hard-won prevention and control achievements, and achieve the goal of "external defense investment and internal defense rebound", recently, the Comprehensive Group of Joint Prevention and Control Mechanism for Epidemic Prevention in novel coronavirus, the State Council issued the Notice on Further Consolidating the Achievements and Improving the Prevention and Control Capability of Medical Institutions in COVID-19.

The notice puts forward requirements for related work from seven aspects:

First, medical institutions are required to improve the management of pre-inspection and triage in outpatient and emergency departments, strictly implement relevant requirements, and do a good job in triage and risk management.

Second, all localities are required to support medical institutions to strengthen the construction and management of fever clinics, ensure that the examination and treatment of fever patients are completed in fever clinics, and prevent cross-infection.

Third, all localities are required to vigorously support medical institutions to strengthen information construction, make appointments for diagnosis and treatment in different periods, reduce the gathering of people on the spot, and do a good job in Internet diagnosis and treatment consultation.

Fourth, all localities are required to strengthen the prevention and control of infection and the protection of medical personnel, increase investment in hospital infection prevention and control from the aspects of funds, personnel and materials, and implement the basic requirements of infection prevention and control.

Fifth, all localities are required to take measures to strengthen the laboratory construction of medical institutions, give priority to supporting the construction of land border ports and other areas, and improve the testing capacity.

Sixth, all localities and medical institutions are required to judge the epidemic situation in advance according to their own characteristics and formulate response plans under different pressures of epidemic prevention and control.

Seventh, all localities are required to strengthen supervision over the prevention, control and treatment of COVID-19 in medical institutions to ensure that all requirements are implemented. At present, the prevention and control of epidemic situation in COVID-19 has achieved initial results. In order to further consolidate the hard-won prevention and control achievements and achieve the goal of "rebound from outside", in view of the gradual progress of the resumption of production, the increasing pressure on the import of overseas epidemic situation, the lifting of the control measures of the Han-E channel, and the existence of certain transmission risks for asymptomatic infected people, the following requirements are put forward for medical institutions to further improve the prevention and treatment capacity of COVID-19:

The first is to improve the management of outpatient and emergency triage.

Medical institutions shall strictly implement the requirements of the Measures for the Administration of Pre-inspection and Triage of Infectious Diseases in Medical Institutions, and do a good job in triage and risk management of patients. Set up pre-inspection and triage points in outpatient and emergency departments, and equip doctors with professional ability and experience in infectious diseases or related majors to enrich the pre-inspection and triage strength. Patients should check their health codes, wear masks, and focus on asking whether they have symptoms and signs of respiratory diseases such as fever and cough, whether they have close contact with patients in COVID-19, and whether they have an epidemiological history of living in areas with high epidemic incidence, and take the patient's temperature. Standardize the transfer of fever patients and highly suspected COVID-19 patients to fever clinics. Conditional areas, combined with the appointment of diagnosis and treatment work, to carry out online first and then on-site two-level pre-inspection and triage.

Second, strengthen the management of fever clinic settings

All localities should support medical institutions to strengthen the construction of fever clinics. The fever clinic should be set up in a relatively independent area in the medical institution, with good ventilation, eye-catching signs, independent toilets, passages and partitions that meet the requirements, and equipped with special equipment and facilities to ensure that the examination and treatment are completed in the fever clinic to the maximum extent. The fever clinic is disinfected regularly, and all personnel wear disposable medical masks for protection. If COVID-19 infection cannot be ruled out through consultation and physical examination, COVID-19 infection should be further ruled out through nucleic acid detection and other related tests. Suspected or confirmed patients in COVID-19 should be registered, reported and isolated by medical personnel in accordance with relevant regulations, and transferred to designated hospitals for further diagnosis or treatment in time, and patients should not be transferred or discharged without authorization.

Third, make appointments for medical treatment and Internet consultation in different periods.

All localities should vigorously support medical institutions to strengthen information construction, carry out appointment registration, appointment examination, appointment for diagnosis and treatment, rationally allocate medical treatment time, realize appointment for medical treatment in different periods, and reduce on-site crowd gathering. Incorporate appointment diagnosis and treatment into the system construction of medical institutions, gradually expand the scope of appointment, and finally realize the goal of "making an appointment before seeing a doctor" for all non-emergency patients. Promote information sharing, effectively identify "four types of personnel" and take targeted measures to reduce the risk of cross-infection. Make full use of the advantages of "Internet+medical care", actively provide online health assessment, health guidance, health education, medical guidance, chronic disease follow-up, psychological counseling and other services, and do a good job in online diagnosis and treatment consultation.

Fourth, strengthen the prevention and control of infection and the protection of medical personnel.

All localities should increase investment in hospital infection prevention and control, and strengthen support and guarantee in terms of funds, personnel and materials. Medical institutions should strictly implement the requirements of the Notice of the General Office of the National Health and Wellness Committee on Further Strengthening the Prevention and Control of Infection in Medical Institutions during the Outbreak and the Technical Guide for the Protection of Medical Staff during the Outbreak in COVID-19. All kinds of personnel entering medical institutions should choose and wear masks correctly, carry out hand hygiene correctly and do a good job in environmental ventilation management. When providing diagnosis and treatment services for suspected and diagnosed patients in COVID-19, additional precautions should be taken on the basis of standard precautions according to operational risks. Medical institutions and relevant responsible persons do not attach importance to the work of sensory control and do not implement the basic requirements of sensory control, resulting in serious hospital infection, which shall be dealt with in accordance with the regulations.

Five, improve the laboratory testing ability of medical institutions.

All localities should take measures to strengthen the laboratory construction of medical institutions, so that all tertiary general hospitals have clinical laboratories that meet the standards of biosafety level II or above. Priority should be given to supporting the construction of land border crossings in areas where medical resources are relatively scarce, laboratory testing capacity is relatively weak and epidemic prevention and control pressure is high. According to the needs of daily diagnosis and treatment, according to the principle of "combining peacetime with wartime", 1 county medical institutions with strong comprehensive strength are selected for key support, so as to realize that medical institutions in the county have the ability of nucleic acid detection. For fever outpatients and patients who are highly suspected of being infected with COVID-19 virus in outpatient and emergency departments, nucleic acid detection should be carried out in time, and all patients should be examined. Encourage medical institutions to take corresponding measures according to the response level and risk level of local epidemic prevention and control, as well as the hospitalization history of hospitalized patients, complete COVID-19 nucleic acid testing and screening before patients are admitted to hospital, and timely find COVID-19 infected people. COVID-19 nucleic acid testing is reported every day in medical institutions. Each province shall submit the examination results of the previous day before 12:00 every day, and the first time is April 13.

Sixth, do a good job in formulating and deducing the response plan

Medical institutions should look for weak links according to their geographical environment and functional orientation in epidemic prevention and control, and formulate response plans under different pressures of epidemic prevention and control. All localities should find out the base of medical resources within their jurisdiction, and accurately grasp the quantity and distribution of medical institutions, treatment beds, instruments and equipment, professional and technical personnel, laboratory testing capacity and protective equipment that can be used for medical treatment in COVID-19. Based on the characteristics of ports, borders and personnel flow in this region, the epidemic situation is judged in advance, and the response plan including personnel deployment, counterpart support, material support and technical support is formulated and deduced on the assumption that there are epidemic situations and outbreaks in different regions. The response plans formulated by all localities at least include determining the starting conditions and response measures of counties, cities and provinces step by step in combination with the pressure of treatment when the number of cases reaches double digits, three digits or even higher, so as to ensure that medical treatment can be carried out quickly and effectively according to the plan when the epidemic occurs. All localities should report the province's response plan to the joint prevention and control mechanism of the State Council before April 20.

Seven, strengthen guidance and supervision.

All localities should strengthen supervision over the prevention, control and treatment of COVID-19 in medical institutions to ensure that all requirements are implemented. To rely on COVID-19 medical treatment expert group, related professional quality control center and other professional forces, in April 20th to carry out special supervision and inspection, timely discover the weak links in the epidemic prevention and control work of medical institutions, guide the improvement work, and promote local medical institutions to prepare for the epidemic. The joint prevention and control mechanism of the State Council will organize supervision and inspection in due course, and the specific time will be notified separately; According to the reported response plan, organize experts to comprehensively analyze and evaluate the prevention, control and treatment of COVID-19 in local medical institutions, and report the evaluation results to the whole country.

All provinces, autonomous regions, municipalities directly under the Central Government and Xinjiang Production and Construction Corps are requested to respond to the joint prevention and control mechanism of COVID-19 epidemic.
