A new method to eliminate varicose veins (April 9, 2003)
.................................................
Hello, audience friends! This is CCTV's live program "The Road to Health". Today we are going to talk about varicose veins.
Moderator: You have treated many patients with varicose veins. What is your biggest feeling?
Liu Peng: What I really want to say to my friends who are suffering from varicose veins is that with the progress of science and technology, especially the birth of new scientific instruments in the last two years, it has brought good news to our patients with varicose veins. Now a brand-new minimally invasive method for treating varicose veins has been widely used in clinic, and I can tell you that ladies who want to be beautiful can put on their skirts again after treating varicose veins with this method.
Moderator: Let's look at the situation of a Ms. Nie: You can't wear short skirts at work. Because of varicose veins, my legs are thick and thin, and the blood vessels in my calves are swollen and look ugly like earthworms. "
Liu Peng: This patient has moderate to severe varicose veins. Let's look at a picture: venous blood flows from the foot to the heart. Normal people's blood does not return after flowing to the heart, while patients with varicose veins have poor blood return, and blood stasis symptoms appear after blood stasis in lower limbs. As shown in the figure, it is a concrete manifestation of varicose veins, which particularly affects the appearance.
Moderator: Let's listen to Ms. Nie again: "It won't take long to walk, it's very painful, and it's very uncomfortable to squat for a long time."
Faye Wong: Patients with varicose veins of great saphenous vein have lost the normal function of blood flowing from the distal end to the proximal end, including the destruction of venous tissue, resulting in relatively high blood vessel pressure, which affects the function of subcutaneous tissue and produces a feeling of soreness. After a long time, the skin will turn black, harden and even fester.
Liu Peng: Generally speaking, varicose veins are superficial veins, and some patients' deep veins are also affected. In patients with deep vein thrombosis, if the thrombus falls off to form pulmonary embolism, the consequences will be very serious, and even life will be taken away in a short time.
Moderator: Let's look at a patient's situation: this is a female patient in her fifties with a medical history of more than 30 years. Some varicose veins on the surface have formed mass changes and feel like sponges. Due to malnutrition on the skin surface, pigmentation will occur, and even foot explosion will occur. The patient's condition is already advanced.
Faye Wong: The course of varicose veins is relatively long. When there are obvious symptoms, including serious complications such as rupture and induration, it is too late to see a doctor. Varicose veins are easy to occur in special people, such as surgeons, teachers, salespeople and other people who often stand.
Liu Peng: Venous varicose veins have several characteristics. For example, they are related to some congenital genetic factors and cannot be prevented. However, some acquired factors can be prevented or should be paid attention to. For example, people who stand for a long time can exercise more at ordinary times, and patients with varicose veins should wear some elastic socks as soon as possible. Varicose veins are a disease that develops gradually and becomes heavier. At present, there is no drug that can radically cure varicose veins, so prevention is very important. Patients with varicose veins have poor blood circulation. If you fly or train for a long time, you must exercise regularly.
Compere: What good method is there to treat varicose veins?
Liu Peng: There are many ways to treat varicose veins, but in the past, patients were generally reluctant to have surgery because of many complications, great pain and too many scars. At present, the new methods are laser cutting and rotary cutting. This method is minimally invasive and can minimize the spots on the legs, which is very beautiful. As shown in the figure, this is a schematic diagram of transparent direct-view rotary cutting. When the light source illuminates the skin, varicose veins can be seen. The rotary cutter can enter through the puncture point of 2-3 mm, and the varicose veins can be sucked away after entering. But this is only a stopgap measure. In order to cure the problem, it is necessary to treat the trunk of great saphenous vein in a targeted way. Lasers can be treated fundamentally. The combination of these two methods is complete, which not only ensures the surgical effect, but also makes the skin very beautiful.
Moderator: Let's take a look at the detailed process of the operation: mark the operation area before operation, make a puncture in the ankle vein after anesthesia, and carry out routine disinfection after puncture. Insert a light source under the skin, so that you can clearly see the varicose great saphenous vein. After inserting the rotary cutting head, the meridians of varicose veins can be interrupted and sucked out under direct vision. Then laser cauterization was performed, and then pressure bandage was applied to stop bleeding, and the operation was completed.
Faye Wong: Patients can go to work immediately after the operation. We advocate that patients can go to the fields early, which can prevent deep vein thrombosis.
Characteristics of laser plus fluoroscopy in direct vision rotation surgery
1. Minimally invasive: There are only 2-3 small incisions of 2-3mm in the calf.
2. The operation time is short: about 20 minutes.
3. Short hospitalization time: 1-3 days or so.
4, beautiful: the legs are basically scar-free.
5, can be carried out under local anesthesia.
6. You can get out of bed after surgery.
7. Less postoperative complications.
Moderator: Let's take a look at Ms. Nie's postoperative situation: Ms. Nie: "The postoperative swelling has gone down, there is no swelling, and there is no scar."
Liu Peng: This is the patient's condition one month after operation. After three months, he will recover better, and there is still some pigmentation on his skin, which will gradually recover. In addition, patients will be more confident.
Moderator: Can recurrent patients who have been treated by traditional surgical methods be treated by this new method?
Liu Peng: Because the traditional method is not thorough and there are many patients with recurrence, it is very suitable to use this minimally invasive method, especially the rotary cutting method, which can completely remove the recurrent and residual veins. All surgical methods have the possibility of recurrence, but this new method is the least prone to recurrence.
Moderator: Ms. Feng from Henan, 33 years old. I have severe varicose veins in both legs. Can we operate together at the same time?
Faye Wong: Of course. Because it is minimally invasive surgery, the operation time is relatively short, and both legs can be operated at the same time.
Moderator: Mr. Chen, 48, from a local hospital in Tianjin, said that my deep venous valve was not functional. Can I have such an operation?
Faye Wong: Patients with deep venous valve insufficiency should be examined by ultrasound before operation. If it is mild to moderate deep venous insufficiency, varicose veins can be removed without surgical treatment for the time being. If it is severe deep venous insufficiency, angiography must be done to judge the degree of reflux. If the degree of reflux is serious, valvuloplasty should be done at the same time.
Moderator: Yu Xiansheng is 36 years old. My father is 58 years old. He has varicose veins for more than 20 years. Now he has an ulcer on his leg. Can he be operated on now?
Liu Peng: It depends on whether the ulcer infection is in the acute stage. If it is in the acute phase, it is necessary to be hospitalized. Use antibiotics to control infection first, and then try surgery. Ulcer patients are not contraindications for surgery.
Moderator: Ms. Shi from Beijing, 56 years old. I have varicose veins for nearly 2 years. Now there are varicose veins in the left leg and foot, and they are getting worse and worse. Can I treat them in this way?
Liu Peng: This situation is especially suitable for rotary cutting, because the traditional method can't do enough.
Moderator: Mr. Zheng is 48 years old. I was treated with great saphenous vein sclerosis three years ago, and now it has recurred. Can you do minimally invasive treatment?
Faye Wong: The treatment of great saphenous vein sclerosis is not widely used now, because it has great limitations and high recurrence rate. It can be treated in a minimally invasive way.
Moderator: Ms. Dai from Shandong, 37 years old. I have varicose veins for five or six years. I feel worse and worse recently, red and itchy. How should I treat them?
Liu Peng: If there is redness and itching recently, it is estimated that the patient has phlebitis at the same time. Go to the hospital to treat phlebitis first, and then do varicose vein surgery after the phlebitis is controlled stably.
