GBZ/T 225-2010 "Guidelines for Occupational Disease Prevention and Control of Employers" 4.1.1Establishing and perfecting the occupational health monitoring files of workers According to regulations, the employer shall establish occupational health monitoring files for workers (including temporary workers) who have labor relations. The roster of workers shall be filed separately before, during and after taking up their posts. The occupational health monitoring files shall include the following contents:-general information such as the name, gender, age, native place, marriage, education level and hobbies of the workers; -Workers' occupational history, past history of occupational hazards and exposure history; —Monitoring results of occupational hazard factors in corresponding workplaces; —Occupational health examination results and treatment; -Workers' health information such as diagnosis and treatment of occupational diseases.
