item description
Encourage all localities to explore the "1+X" physical examination mode. Among them, "1" is a basic and necessary physical examination item. "X" is a self-selected item for all localities, which can be a health check-up package dynamically designed by all localities according to the physical examination results of the local area or the previous year, or a personalized physical examination item selected by residents according to their own conditions. The specific implementation measures shall be determined by all localities themselves.
If the physical examination object is inconvenient to travel or move, and the physical examination institution needs to provide physical examination services at home, with the consent of the county-level health administrative department, other laboratory examination items can be used instead of chest X-ray or DR examination without increasing the medical examination funds.
Physical examination institutions should co-ordinate arrangements for health examination, women's "two cancers" examination, colorectal cancer screening and other projects to facilitate residents to participate in physical examination nearby.
Feedback of physical examination results and health management
After the physical examination, the physical examination institution should analyze and evaluate the physical examination results, issue a health examination report (book), and feedback the written physical examination results to the examinee within one month (immediate feedback of critical value). The contents of the health examination report shall include the examination results, comprehensive evaluation and health guidance of the physical examination items.
Suspected cases of chronic diseases, infectious diseases, tumors and other diseases found in physical examination should be further examined or referred in time, and the diagnosis results should be tracked. Physical examination institutions should organically combine health examination with community health management, and bring the high-risk groups of chronic diseases and confirmed patients found in physical examination into the corresponding diseases for standardized management, so as to provide sustained and comprehensive health services.
Physical examination information management
The medical examination institution shall keep the personal information of the medical examination object confidential. After the physical examination, the physical examination institution shall promptly import the physical examination information into the resident electronic health record system. Physical examination objects that have not established residents' health records should be filed as required.
After the physical examination, the physical examination results are combined with the residents' electronic health records and made public to the residents through the mobile phone client of Li Zhe Office.
Medical examination funds and management
The subsidy standard for basic items of health examination for urban and rural residents in the province (except children aged 0-6 and primary and secondary school students) is 90 yuan/person-time. Medical expenses for basic projects shall be arranged by financial departments at all levels. According to the local economic and social development level and financial affordability, all localities can appropriately increase physical examination items and raise subsidy standards on the basis of basic physical examination items.
The medical expenses of optional items shall be borne by all levels of finance and individual urban and rural residents, and the financial part shall not be higher than 60 yuan/person in principle. Where the local original standard is higher than this standard, the original standard can continue to be implemented.
Finance at all levels can co-ordinate other basic public health service subsidy funds for health examination. Strengthen the overall planning of physical examination, improve the sharing mechanism of physical examination information, and ensure that individuals enjoy health examination according to the principle of high physical examination standards. The number of free health examinations provided by the government does not exceed 1 time every year.
In areas where the annual physical examination is carried out for people over 60 years old, the insured residents who have reached the age of 60 and participated in the annual physical examination as of 202 1,1,3 1 can continue to implement according to the population policy of 65 years old and above.
