Application material
Candidates are required to submit the following materials:
1. Personal appraisal application form: signed by the candidate himself, and the registration information shall not be changed after signature confirmation. Unsigned registration materials are invalid.
2. Graduation certificate: original and photocopy. Candidates with college education or above are required to submit the Xue Xin Electronic Registration Record Form. Com diploma.
3. ID card: original and photocopy.
4. Photos: 2 photos of 1 inch (jpg format, size between 15kB-45kB).
5. Training certificate: only candidates who are not medical and health professionals and have medical and health secondary school education are required to submit.
Copies of all materials shall be submitted in duplicate.
Registration process
1. Individual registration of candidates: submit paper application materials to the appraisal station, package and send the electronic version of the candidate information form (Annex 2) and 1 inch photos to the designated mailbox of the appraisal station. 2. Registration of training institutions: submit all candidates' paper registration materials to the appraisal station, and package and send the candidates' electronic information form and 1 inch photos to the designated mailbox of the appraisal station. Please fill in the subject of the email as? Name of training institution+appraisal type of work? . What is the electronic photo of each candidate? Id number? Naming, the candidate information form must be filled in completely.
