1. Original and photocopy of my second-generation ID card or household registration book (check the original and receive the copy);
2. Employment certificate. Any of the following forms of proof is acceptable, and the employment permit must be stamped by the issuing unit:
(1) Notice of Preventive Health Inspection issued by the district and county market supervision bureau, district and county new bureau of culture and light, district and county health supervision and law enforcement brigade and other relevant departments;
(2) Business license of the institution;
(3) The work certificate of the employing unit or the unit to be appointed.
The processing flow of health certificate is as follows:
1. Bring the above-mentioned handling materials to the local medical examination unit for handling;
2. Get the medical examination form and pay the medical examination fee at the toll booth;
3. Laboratory blood collection and stool anal swab test;
4, internal and surgical routine physical examination;
5, X-ray chest X-ray;
6. After completing all physical examination items, those who pass the physical examination will get a health certificate within 3 to 7 working days.
To sum up, it usually takes about a week to get a health certificate. Food production and marketing personnel who are in contact with directly imported food or directly serve customers in public places need to carry ID cards and other materials for health examination and obtain health certificates before they can engage in related work. In addition, if food producers find that the food they produce does not meet the food safety standards or there is evidence that it may endanger human health, they should immediately stop production, recall the food that has been put on the market, notify the relevant producers, operators and consumers, and record the recall and notification.

Food producers and business operators shall establish and implement the health management system for employees. Persons suffering from diseases that hinder food safety as stipulated by the administrative department of health of the State Council shall not engage in direct contact with imported food.
Food production and marketing personnel engaged in direct contact with imported food shall undergo annual health examination and obtain health certificates before taking up their posts.
