Fifteenth social welfare institutions should sign a service agreement with the clients or their families (guardians) to clarify the responsibilities, rights and obligations of both parties.
The establishment of social welfare institutions for orphans and abandoned babies by social organizations and individuals must be jointly organized by the civil affairs departments of local people's governments at or above the county level; The adoption of orphans or abandoned babies by social welfare institutions shall be examined and approved by the competent civil affairs departments one by one, and an adoption agreement shall be signed.

Article 12 To apply for the establishment of an old-age care institution, the following documents and materials shall be submitted to the licensing authority:
(1) An application for establishment;
(two) the qualification documents of the applicant, the proposed legal representative or the principal responsible person;
(3) The name, articles of association and management system of the institution that meets the registration requirements;
(four) the completion acceptance certificate of the construction unit, the acceptance report or review opinion of the health and epidemic prevention and environmental protection department, the fire design review and fire acceptance opinion of the construction project issued by the public security fire department, or the fire record certificate;
(five) the certificate of ownership of the service place or the house lease contract;
(six) the list of managers, professional and technical personnel and service personnel, identity documents and health certificates;
(seven) other materials required by laws, regulations and rules.
To apply for the establishment of an operating pension institution, a business license and a copy thereof shall also be provided.
