I. Reporting conditions

(4) have a well-established constitution, and the name of the organization shall comply with the regulations and requirements of the registration authority;
(5) have management and service personnel appropriate to the services they provide, and the medical personnel shall comply with the qualifications stipulated by the administrative department of health, and the nursing personnel and staff shall meet the health standards set by the relevant departments.
II. Reporting Procedures
(1) Submit the following materials to the civil affairs department of the people's government at or above the county level where the welfare institution is located to submit an application for preparations for running the institution:
a, an application form and a feasibility report;
b, documents proving the qualifications of the applicant;
c, documents proving the source of funding for the proposed social welfare institution;
d. Documents proving the fixed place of the proposed social welfare organization.
(2) The civil affairs department makes a decision on whether to agree to set up or not to agree to set up a welfare institution according to the criteria for setting up welfare institutions, and notifies the applicant in writing.
(3) When a welfare institution agreed to be set up has the conditions for opening, it shall apply to the civil affairs department for a Certificate of Approval for the Establishment of Social Welfare Institutions and provide the following documents:
a. A written report applying for a Certificate of Approval for the Establishment of Social Welfare Institutions;
b. A Certificate of Approval for the Establishment of Social Welfare Institutions issued by the civil affairs department;
c, Proof of ownership or rental contract of the service premises;
d, acceptance report or examination opinion letter from relevant departments of construction, fire fighting, health and epidemic prevention, etc.
e, proof of capital verification and asset evaluation report;
f, articles of incorporation and rules and regulations of the organization;
g, lists of management personnel, professional and technical personnel, and caregivers and copies of their valid documents, as well as health conditions of the staff. photocopies, and certificates of health status of the staff;
h, other materials requested.
(4) Register with other registration authorities.
