5, shift time: refers to the specific shift time, if necessary, recorded to the hour, minute. Signature: The full name should be signed to show responsibility for the child.

Third, the parent contact book (Form 13) fill in the requirements:
1, shift: the name of the child's class. Date: the specific time of contact with parents.
2. Child's name, parent's name: just fill in the truth.
3, contact methods: truthfully fill in, such as individual interviews, letters, seminars, telephone contact.
4, the content of the contact: fill in the purpose of contact with parents or the need for parents to cooperate with the solution of the problem. Such as ① children's poor health habits; ② frail children's management of home cooperation; ③ shortcomings corrective home cooperation; ④ children's physical and mental development of the problem; ⑤ children's parents of health care work suggestions.
5. Evaluation of the effect: the effect should be judged by whether the contents or problems of the contact have been solved and whether the expected purpose has been achieved.
Four, class utensils disinfection register (Table 19) fill in the requirements:
