The drama is a co-production between Fuji and ****Taiwan and stars Yuji Oda and Ken Ishiguro.

The main supporting cast includes Yuki Matsushita as Dr. Peak, Aiho Chidori as Dr. Chikako, and Yuki Kagawa as Dr. Chikako.

Josefumi Kaga (as Director Nakagawa), and Nakamura Azuchi (Hoshino Ryoko).

The work is a bold, detailed exposé of the dark side of Japanese medicine (e.g., not saving a patient if he's not on duty; doctors sleeping in the CT room when no one's there; gambling on who's quicker to operate; in short, it's darker than you'd

dare believe. In the midst of controversy and confrontation, it indicts the one-sidedness of Japanese society, hypocrisy and some

unfair and unreasonable world conditions.

Episode Plot

The First Story

A new doctor, Gen Ishikawa, returns from Kansas, USA, to Tenjinro Hospital. On the day he reports for duty, an emergency situation arises. First, a resident, Kashiwagi, disappears, and then two emergency patients need to be operated on at the same time, but Harumi Mine, who is on duty, is only an intern; the head of the surgical department, Junichi Nakagawa, has been invited by Hoshino of the Ortec medical equipment company to have a social gathering; and the only remaining surgeon, Eotaro Sima, refuses to operate on the grounds that he is off duty. The only surgeon who remained at the hospital, Eitaro Sima, refused to operate on the grounds that "I'm already off duty," and said, "Let's let the patient die if he can't be saved. Ishikawa had no choice but to perform both surgeries at the same time. At that moment, the dean of the hospital called to tell him that the son of a councilor had been in a car accident and needed to be rescued. Just when Ishikawa was distracted, Sima took over the surgery.

The Second Story

The surgery department receives a critical patient with a myocardial infarction, and Ishikawa and Sima argue over the treatment plan for this patient. Ishikawa advocates for conservative treatment, while Sima believes that conservative treatment is equivalent to letting the patient wait for death, and argues for taking the risk of operating on the patient even if there is only a 10% chance of success. While Ishikawa's opinion is approved, Sima uses Hoshino to bribe an anesthesiologist to perform the surgery without authorization. When Ishikawa realizes this, the two argue in the operating room.

Minister Nakagawa changes his mind and decides to let Sima risk the operation. The surgery goes well, however the patient passes away at the last minute due to a low vital index. After work, Sima stood alone and silently beside the patient's body covered with a white cloth, when the ECG, which the nurse had forgotten to turn off, suddenly reacted, Sima thought for a moment and went up to turn off the instrument.

The Third Story

Dr. Feng told Dr. Sima that he had turned off the EKG. The surgical department decides to set up a "punishment committee" to punish Sima. Ishikawa and Mines ask Sawako Oshikaga, an anesthesiologist and Sima's ex-girlfriend, to testify about the incident, and find the patient's ECG records of that day with the help of Kan Inamura, a social worker at the hospital. At the punishment committee, Minister Nakagawa intends to take Sima's side, but Sima is forced to admit to the fact that he turned off the reactive EKG machine in the face of ample evidence. At that moment, nurse Megumi suddenly came forward and said that she had turned off the EKG.......

Fourth Story

A patient with a large abdominal aneurysm was brought to the hospital, and Chief of Surgery Hiraga thought that the situation was very dangerous and that the patient needed to be operated on right away or else the patient would be in danger of dying if the aneurysm ruptured. However, the patient himself is the president of a company, and he has to attend a meeting that night that will affect the company's survival. A subordinate who brought the president to the hospital asked the hospital to hide the truth from the patient and delay the operation. Sima accepts the bribe and agrees to the request, and finds an X-ray of a herniated lumbar disc to use as a pretext.

Ishikawa found something fishy about the X-ray and, after investigating, discovered that Sima had tampered with it. He comes to the patient and tells him what happened, but the patient himself decides to attend the meeting that night with Sima's assurance that "I will save your life".

The patient fainted at the end of the meeting, and Peak, who had been at the scene, rushed the patient to the hospital. Minister Nakagawa decides to give the operation to both Sima and Ishikawa. During the operation, Sima deliberately cuts Ishikawa's hand with a scalpel because he thinks Ishikawa is too slow.

The Fifth Story

Mr. Nakagawa wants to promote Ishikawa to the position of Counselor, but Sima is not happy about it. A gas explosion occurs in a densely inhabited area and a number of patients are brought to the hospital, where Ishikawa directs the doctors to treat the patients individually. One of the patients, Ms. Ueno, is brought to the hospital in good spirits and is ignored by Ishikawa, who waits in the treatment room for a long time. When Ishikawa finishes treating the other patients, Sima finds Ueno collapsed in the corridor.

A CT scan shows that Ueno's injury, a pelvic fracture causing damage to the aorta, is irreparable, but Sima decides to operate on him. Ueno died and his son arrived at the hospital in the pouring rain after a phone call from Sima. Ishikawa feels guilty about this ......

Sixth Story

Because of Ishikawa's mistakes, Nakagawa re-promotes Sima to councillor and promotes him to chief of surgery, demoting Hiraga to deputy chief. Hiraga is y jealous of this. Ishikawa has been mentally unstable for his mistakes, and Inamura suggests that he take advantage of his vacation to have a physical examination. At this time, a special patient is admitted to the hospital - actress Hitomi Fujisaki, who suffers from a stomach ulcer.

Sima was secretly pushed down the stairs by a jealous Hiraga, resulting in a sprained right arm. At this time, Hitomi Fujisaki has a complication of cholecystitis and needs an operation, but due to her special status, she can't be operated on, so Sima proposes the difficult operation of "laparoscopic gallbladder removal", but due to her injury, she can't do it by herself.

Nakagawa agreed to perform the surgery himself, but his right hand trembled when the surgery started, so he pretended to faint and withdrew from the surgery. Sima enters the operating room with his injury and tries to guide Dr. Peak in the operation, but Ishikawa arrives to take Peak's place and completes the difficult surgery with just a few simple hints from Sima.

Seventh Story

When Dr. Yamamura was reviewing the X-ray film of the medical checkup, he noticed that Ishikawa's film actually had a shadow on his stomach, he approached Otsutsuki to discuss the matter, and Otsutsuki told Sima about it, and asked Sima to never tell Ishikawa about it. The doctors decide to collectively hide the truth from Ishikawa, but Ishikawa himself is increasingly skeptical about the results of the "gastric ulcer" test.

Seoka, a terminal cancer patient, is playing the black pipe in the ward. The nurses and patients can't stand it and complain to Inamura, but Inamura thinks that Seoka's behavior is the life support of a dying man, and urges everyone to be patient. Szema approaches Hoshino about purchasing medical equipment and blatantly asks for a kickback. Dr. Maegano finally can't stand Seioka's blowing and stops him loudly, stating that it was Director Sima's order. Seioka is extremely upset and runs to the roof of the building alone to cry in the dark. Ishikawa learns of this and angrily goes to Sima to argue about it, only to be told by Sima that "Instead of investigating other people's business, you should take care of yourself."

The Eighth Word

Sima tells Ishikawa directly that he is suffering from stomach cancer, and that it is quite serious. Ishikawa is shocked, and after careful consideration, asks Minister Nakagawa to let him work until he can't hold out any longer. Tenjinro Hospital receives a large-scale operation, "Extracorporeal Liver Transplantation", which requires four teams of doctors to perform the operation, and the reputation of the hospital is at stake. Minister Nakagawa asks Sima to lead the operation, and Sima is determined to break the record for this type of surgery in terms of time.

Hoshino comes to see Sima to sell his company's equipment and promises to give him a 35% rebate. While looking through the catalog, Sima accidentally finds the name of a German company that sells NRD (a radiation device for cancer treatment with low side effects), but Hoshino's company does not have the right to represent NRD at the moment, so Sima agrees to buy all of their products on the condition that they introduce NRD.

Osatsuki inadvertently discovers the secret of Nakagawa's trembling right hand, and thus guesses that something has happened between Nakagawa and Sima. The operation conducted by Sima managed to break the Japanese record with a time of 9 hours and 50 minutes, and a large photo was published on the front page of the newspaper the next day, making the Tenjin House Hospital a sight to behold.

Sentence 9

On the roof deck, Sima finds Seioka and apologizes for stopping him from blowing a black pipe a while ago. Seioka says that he doesn't hold a grudge against Sima. At the same time, he tells Sima that he feels that he will die soon, and hopes that he won't suffer too much when he dies, and that he doesn't want to drag his family down anymore. Sima suggested to him that he should die with dignity.

Sima uses the money Hoshino gave him to bribe both Nakagawa and Hiraga, and threatens to tell the truth about his inability to operate if he doesn't obey him, which Nakagawa finds hard to say.

Ishikawa is surprised that Sima buys all of his medical equipment from Otter and suspects that Sima is receiving kickbacks from Otter. He asks Minister Nakagawa to report Sima's illegal behavior and finds Hoshino to ask her to testify about the bribe in exchange for long-term purchase of Otter's products. At the purchase meeting, Nakagawa reveals that Sima received the bribe and takes out the money Sima handed him. However, during a break in the meeting, Sima bribes the chairman of the hospital's board of trustees with a large sum of money. Subsequently, during the confrontation with Hoshino, both parties knowingly led the matter to Hiraga, who received the money for Sima, and Nakagawa went along with it, Sima passed the test, and Hiraga was fired from the hospital as a scapegoat.

Inamura advises Ishikawa to cherish himself and Peak, who likes him, and not to waste any more time working against Sima.

The Tenth Story

Ishikawa's surgery is scheduled for three days later, and Nakagawa decides that Dr. Maeno will operate on Ishikawa.

Seoka's condition deteriorated, but he did not have time to make a written request to the hospital for a "death with dignity" before the attack. Sima looks at Seok-gon, who has tubes all over his body, and feels his pain, which is worse than death.

Ishikawa believes that as long as the patient's family has the will, doctors should do their best to prolong the patient's life. Sima, however, says that saving his life, and keeping him from dying, are not the same thing.

In the hospital room, at the pleading of Seok-gon's eyes, Sima dismisses the nurse and injects Seok-gon with petararufan, and Seok-gon passes away with a contented smile.

Nurse Chiyo told Ishikawa about Sima leaving the petararufan injection in the hospital room, and Ishikawa took it as his last chance to bring Sima down. Not only did he tell the truth to Seoka's family, but he also contacted the news media. The hospital administration has a big headache over the matter, and Minister Nakagawa and the chairman of the board of directors agree that they can no longer cover up Sima's behavior, and Sima is forced to resign. Ishikawa finally reaches his last goal and immediately afterward vomits blood and faints in his office.......

Final Words

At the entrance of the hospital, Sterling calls out to Sima, who is about to leave, and asks him to save Ishikawa. Sima asks Nakagawa to operate on Ishikawa with him as the lead surgeon, and Nakagawa agrees to this, while Ishikawa himself refuses to sign the surgery protocol.

After much persuasion from Feng, Ishikawa finally agreed to let Sima operate on him. Sima told Ishikawa that the possibility of saving him in his current situation is basically zero, but asked Ishikawa himself to raise the possibility of survival to 10% with his desire to live, and he will try to raise it to 20% as a doctor. In the operating room, Sima and Feng fight hand in hand, and after several hours of hard work, the surgery is successfully completed. A relieved smile finally appears on Sima's face. When Ishikawa awoke, the two men's hands clasped together.

Sima packed up his things and prepared to leave the hospital, and just at this time, Ishikawa suddenly suffered a pulmonary obstruction, and Sima rushed to the ward to do his best to save him, but he was finally unable to return to life. On the winter street, leaving the hospital, Sima walked aimlessly, when Hiraga, who was expelled from the hospital, quietly leaned behind him ......
