Coal Mine Accident in Langshui, Guizhou Province

At about 11:00 p.m. on November 24, 2012, a coal and gas outburst accident occurred at the Xiangshui Coal Mine of the Guizhou Panjiang Group, which is located in Panxian County, Liupanshui City, Guizhou Province. Twenty-eight people were underground at the time of the accident. As of 17:30 on the same day, the accident has caused 18 deaths, five people were rescued, another five people are still trapped underground, rescue workers are searching for them.

The project manager of the Ringshui coal mine is Guizhou Pannan Coal Development Co. The company is led by Guizhou Panjiang Coal and Electricity Co., Ltd, together with Yankuang Guizhou Energy and Chemical Co., Ltd, Guizhou Yueqian Electric Power Co., Ltd, and Guizhou Bureau of Coalfield Geology invested in the establishment of the company When asked how to build the Ringshui Coal Mine into a safe, efficient and modernized enterprise, Li Zongxi, chairman of the board of directors, said: "Intrinsic safety, harmonious development, resource conservation, and quality and efficiency are the basic requirements of modernized mine, and it is our basic requirement for coal mine to be safe. The basic requirements for a modernized mine are essential safety, harmonious development, resource conservation, quality and efficiency, which are the concrete embodiment of the scientific concept of development for our coal mining enterprises, and must be taken as the goal to be pursued. Modernized coal mines, as coal production enterprises, should pursue quality and efficiency, but at the same time, they should also focus on social responsibility and build a responsible enterprise, but also make efforts in creating learning enterprises, cultural enterprises, innovative enterprises and other aspects. If you don't do this, the enterprise will have no vitality, it will not be able to stimulate the entrepreneurial passion of the workers, and the enterprise will not be able to obtain long-term development."

Since the construction of the mine began in December 2003, the mine has gradually established and perfected 138 responsibility systems covering all positions in the company, including 76 safety production management systems and 216 management rules against "three violations"; it has also formulated safety operation procedures for 128 types of work in five categories. Ringshui Mine also relies on scientific and technological progress, increasing safety investment, focusing on gas management, preventing gas protrusion as the focus of safety improvement, and improving the mine's ability to resist disasters. The use of physical exploration, three-dimensional seismic exploration and other advanced means of geological data provided by the geological department has not been explored in the geological structure and coal seams of the situation to carry out supplementary exploration, to provide technical support for the sustainable development of the mine.

After the integration of Liuzhi Mine by Panjiang Group, the coal resources on hand have increased, and may be injected into the listed company in the future, but the time is not ripe now. At present, the most likely to be injected into the group's holdings of the Ringshui coal mine, the market has been concerned for a long time. 09 March Panjiang shares completed the acquisition of assets of the controlling shareholders, through the directional issuance of injected into the Tucheng mine, the moon field, the foot of the tree and Jinjia mine and other major mining assets. These injected assets are 100% owned by Panjiang Group. All the coal assets except for the Langshui Mine have been injected into the listed company. At the time of the injection, the coal mine was already in production, with an output of 1.32 million tons in 2008, and now has a production capacity of 4 million tons/year, with a long-term plan to reach 10 million tons/year. However, its complicated shareholding structure may hinder its injection. The direct owner of Ringshui Coal Mine is Guizhou Pannan Coal Development Co., Ltd, of which the Group's subsidiary Panjiang Coal and Electricity Co. accounts for 36%, Yanzhou Guizhou Energy Chemical Co. accounts for 27%, Guizhou Yueqian Electricity Co. accounts for 27%, and Guizhou Coalfield Geological Bureau accounts for 10%. At 23:20 on December 24, 2005, a fire accident occurred on the upper mountain of the belt conveyor of the No. 19 seam of the broadcasting soil mining area of the Pingshui coal mine of the Pannan Coal Development Co. in Guizhou Province, during the construction of the infrastructure from the top to the bottom, and the whereabouts of the 12 people underground were unknown, and it was estimated that they had no possibility of survival.

It is understood that the mine is a state-owned mega mine under construction, by Guizhou Panjiang Coal and Electricity Company Limited, Yankuang Guizhou Energy and Chemical Company Limited, Guizhou Yueqian Electric Power Company Limited, Guizhou Provincial Bureau of Coalfield Geology four state-owned companies *** with the construction of capital. The construction of the mine started in 2004, with a designed production capacity of 10 million tons/year, and was constructed in two phases, the first phase with a designed capacity of 4 million tons/year, and the second phase with a designed capacity of 6 million tons/year, which is a coal and gas prominent mine. The mine ****designed four uphill, respectively, No. 17 coal seam return wind uphill, No. 17 coal seam belt uphill, No. 19 coal seam belt uphill, No. 19 coal seam track uphill. The accident occurred in the No. 19 coal seam belt uphill, which is designed to be 1,631 meters long, started construction in March 2004, and has been constructed for 1,381 meters, with a roadway section of 18.3 square meters, and a layer of 0.72-meter-thick coal seam at the top of the roadway. The upper hill is constructed by China Railway Construction Corporation 19th Bureau Group Third Engineering Co., Ltd. in the direction of the lower hill. The company has the qualification of mining engineering construction. Preliminary analysis shows that the cause of the accident was the prolonged protrusion of coal and gas, which caused gas combustion after the discharge of guns in the upper mountain of the belt transportation of the No. 19 coal seam in the broadcast soil mining area of the mine.

At the critical moment when all parts of the country are conscientiously implementing the "Notice of the Office of the Minister on the Safe Production Work in Winter and Spring" (Mingdian (2005) No. 32), increasing safety supervision and ensuring the safe production of coal mines in winter and spring, the "12.24" accident occurred in the construction of the mine in Rongshui Coal Mine in Guizhou Province, which indicates that the construction of the mine is not in the best interest of the people. The "12.24" accident occurred in the construction of the mine, indicating that the safety responsibility system was not implemented and there were serious loopholes in the safety management during the construction of the roadway. The relevant departments in Guizhou Province should order the mine to immediately stop construction and rectify the situation, quickly identify the causes, seriously investigate the responsibility according to the law, and publicly deal with and accept social supervision.

In order to seriously absorb the lessons of the accident, and effectively strengthen the safety of infrastructure mines, to prevent similar accidents from recurring, the following requirements:

One, strict market management of coal mine construction projects. To establish a strict market access system for coal mine construction construction enterprises, all enterprises engaged in the construction of coal mine construction projects must have the appropriate construction qualifications and obtain a safety production license, with a sound safety management institutions and strict construction standards, to meet the safety needs of coal mine safety management personnel and technical personnel. To regulate the market behavior of coal mine construction projects, strict bidding system, and resolutely prevent unhealthy competition and low price after winning the bid to reduce engineering standards, change the design program, cut safety investment, subcontracting projects and employment of contractors construction and other behavior.

Two, effectively strengthen the coal mine construction construction enterprise safety management. To strictly in accordance with the "Coal Mine Safety Regulations" and "Coal Mine Construction Safety Regulations" of the relevant provisions of the organization of construction, the establishment of internal production safety responsibility system, strengthen the safety of construction management, establish and improve the management system; the establishment and implementation of the accident hidden danger investigation, rectification system, seriously rectify the construction process may exist in the safety hazards, and resolutely put an end to the violation of the rules of the command, illegal operation, violation of labor discipline, to ensure the Safe construction.

Three, strengthen the ventilation gas management and fire prevention and suppression management in the construction process of the shaft. To ensure that the digging face has enough wind, with full-time gas inspector, strengthen gas inspection, strictly prohibit empty shifts missed inspection; strict management of gunning and pyrotechnics, strict implementation of the "one gun, three inspection" and "three interlocking" gunning system, strictly prohibit open-flame gunning and the use of explosives, detonators that do not comply with the regulations, and the use of the fire. Four, increase the supervision of coal mine construction projects. At 23:12 on December 24, 2005, a coal and gas protrusion accident occurred in Pingshui coal mine in Panxian, Guizhou Province. 23:20 the protruding gas washed out of the wellhead and spread, and when it encountered the ground ignition source, it caused the gas to burn. The fire and rescue scene at the wellhead of the accident is shown in Figure 1-5. The accident killed 12 people and caused direct economic losses of 3.546 million yuan. The amount of coal protruded was 2,500 tons, and the gas protruded was about 1.1 million m3.

I. Situation of the Accident Mine and the Accident Area

The Ringshui Coal Mine is designed to have an annual production capacity of 10 million tons, and it is divided into two phases of construction: the first phase is designed to have an annual production capacity of 4 million tons; and the second phase is designed to have an annual production capacity of 6 million tons. The first phase is divided into Hexi Mining Area and Baitu Mining Area, of which Hexi Mining Area is designed to have an annual production capacity of 1 million tons and Baitu Mining Area is designed to have an annual production capacity of 3 million tons.

The accident happened in the 19# coal belt uphill of the broadcast soil mining area, when the accident happened, the 19# coal belt uphill of the mining area has been dug 1,381m, the 17# coal track uphill has been dug 1,208m, and the 19# coal track uphill has been dug 1,448m. the dug roadway of the broadcast soil mining area of the Ringshui Coal Mine is shown in Fig. 6.

In the process of digging, it was found that the geological structure changed from 1199 meters, and the inclination angle of the coal seam changed from 9 degrees to 12.6 degrees.

The 19# coal belt uphill is ventilated by 2×45kW local ventilators and 800mm ducts, and the return air volume of the shaft is 500m3/min. There is one gas sensor for each head and return air volume, and the wind power and gas electric locking device are in normal use.

Second, the accident occurred

December 24, 2005, 19 # coal belt uphill day shift normal digging, to 19:00 has completed a cycle of operations, the head of the slag, stenting, anchors, and other processes have been completed.

At 19:00, the night shift duty team leader presided over the night shift pre-shift meeting, arranged for the normal digging footage of 1.8m on the shift. 12 people entered the well operation on the shift*** (including 3 people pumping, 5 people drilling and firing, and 4 people slagging out), at 23:00, the gunnery worker called the wellhead duty room and said that he would like to fire the gun, and the duty team leader answered the call and agreed to fire the gun, and arranged for the checking worker to cut off power and lighting power supply at the ground level. 23:12. At 23:12, the well was fired, and the sound of firing was heard at the entrance of the well. A few minutes later, a gust of wind blew out from the well, accompanied by black dust, and the wind was getting stronger and stronger, so the team leader and the body checker went to the entrance of the well to observe the situation, and when they were about 5 meters away from the entrance of the well, they were knocked down by the strong wind blowing out of the well (they were not injured), and they then ran back. Ran out about 30m, looking back, found the wellhead appeared open flame flame, fire is very fierce, flame up to 40-50m. small team leader immediately called the duty captain to report the accident.

Third, the accident rescue

(a) the mouth of the closed

After the accident, the resonant coal mine set up a rescue and relief headquarters, formulated rescue measures, and immediately carried out various rescue and relief work. The first request Panjiang rescue brigade and Panxian fire department to participate in the rescue. At the same time, the accident was reported to the higher authorities in accordance with the prescribed procedures.

After receiving the report, the provincial safety and obscene will immediately start the mine accident emergency plan, sent personnel to the accident scene to direct the rescue work. At the same time, additional six Zhi rescue brigade and call the provincial fire brigade to participate in the rescue.

At 14:00 on the 25th, the relevant departments and units rushed to the scene, re-adjusted the command at the scene, while the command meeting was held to analyze the accident and study the accident treatment program. According to the scene, the wellhead flame up to 40-50m, the fire is too big, the personnel can not get close, can only be observed in 40m away. Due to the high concentration of underground gas supply, the fire has not been reduced, analyzing the possibility of coal and gas protrusion, the protruding gas meets the ground fire source caused by combustion. Since the upper mountain has only one exit which is the same as that of the ground, and has been blocked by the open fire, it is impossible to supply air to the underground to dilute the gas. The underground tunnel has been under the control of high concentration of gas for a long time, and the trapped people lack the basic survival conditions, and there is no possibility of survival for the trapped people in the well. Command believes that the fire is too large, the use of direct fire suppression can not be effective. According to the actual scene, the command developed a rescue and relief program: 1, first closed the wellhead with soil, to meet the conditions can be opened and closed the fire area for treatment; 2, set up a cordon by the rescue team at the scene 50m range, monitoring the wellhead range gas, and directing the mechanical bulldozing to seal the wellhead to extinguish the fire; 3, mobilizing dump trucks, loaders, excavators, and other construction machinery, from the wellhead of the place of 1,000m from the soil to be transported to the Wellhead near the pile; 4, the soil gradually pushed to the wellhead, pile blocking the wellhead to isolate the air, and at the same time the use of high-foam fire-fighting technology to extinguish the fire, and ultimately achieve the closure of the fire area.

In the early morning of the 26th, the use of construction machinery to close the wellhead of the soil required to be ready; 9 o'clock began to close the operation. 10:15 wellhead safety closed, the fire is basically under control.

Because of the high temperature of the wellbore, after the closure of the wellbore gas pressure increases, gas spillage along the gaps in the soil, in the sealing of the surface of the soil there are still some sporadic embers. Command decided: by the mine rescue team wearing breathing gas, to take from the pile of soil around, from the bottom to the top of the water while tamping soil method, gradually shrink the scope, and eventually extinguish the open flame flame.

February 16, after the closure of the gas, temperature and other observation data since the analysis, the fire in the shaft has been extinguished, in line with the conditions and provisions of the opening and closing of the fire zone, the command decided: open and close the disaster area. The opening and sealing work by the mine rescue according to the provisions of the "Coal Mine Rescue Regulations", the development of reconnaissance programs and measures to implement, construction machinery with.

At 10:50 on the 16th, the sealing began, and at 16:30, the entrance to the shaft was first dug open to a 0.5 m2 entrance, and wind curtains were set up with wind barriers to lock the wind and prevent the ground air from entering the disaster area.

(C) reconnaissance of the disaster area

16:30 reconnaissance personnel into the well, 18:26 reconnaissance ended, and reported to the rescue headquarters to reconnaissance;

In 900m found two people killed, the direction of dumping down, the CH4 concentration of 90%, O2 concentration of 0.7%, no CO, T is 170C.

3, shaft down about 1030m to avoid the hole found three people killed, the direction of dumping up 2, down 1, wind pipe, cable, etc., intact, there are no traces of explosion. Here there is a protruding coal dust about 0.5m thick, the wind pipe has 20 sections piled up together (for the protruding rushing rolls), there is no trace of burning; the concentration of CH4 in this place is 90%, the concentration of O2 is 0.7%, there is no CO, and the T is 170C.

4. 1 person was found dead in the sheltering hole at 1120m from the wellhead, and there is no traces of explosion and combustion. There is a protruding coal dust about 0.7m thick, the concentration of CH4 is 90%, the concentration of O2 is 0.7%, there is no CO, and the T is 170C.

5. 1200m away from the wellhead, the downward direction has been flooded (the distance of flooding to the downward direction is about 181m), and there is a 0.5-1.5m thick coal dust in the upward direction of the flooding in the range of about 100m distance.

6, the above found six victims have no burn marks, judgment is asphyxiation death.

(4) Restore ventilation, carry and find the victims

The command decided: firstly, discharge the gas, restore ventilation, then carry the found victims, and finally clean up the roadway to find the rest of the victims. In order to prevent coal dust explosion when cleaning up the protruding objects in the roadway, the rescue team should take water sprinkling and dust prevention measures when cleaning up the roadway.

1, February 16, 21:40, the rescue team in accordance with the measures to discharge gas, by discharge gas, at 6:00 on the 17th, the mouth of the well to 1200m gas discharge is completed, ventilation resumed.

2. At 16:25 on the 17th, the ambulance team carried the 6 victims out of the ground.

3. At 9:00 on the 18th, they started to restore underground power supply, resume belt operation and water pumps for drainage, continue to clean up protruding objects, and search for the remaining 6 victims.

4. At 3:50 on April 22, the last person in distress was found at 1280m and transported out of the ground, (see Figure 6, shown in the schematic diagram of the excavated roadway in the broadcasting soil mining area of the Ringshui Mine) the rescue work was finished.

On June 10, the work of cleaning up the roadway was completed.

Fourth, the cause of the accident analysis

(a) the direct cause

1, according to the site investigation, the wellhead down 50 ~ 1200m range, the cable, duct, belt, pumps, etc. intact and gas parameters, according to the preheating - combustion - coking process analysis, in this range, no traces of carbide and combustion explosions were found. Carbide and traces of combustion and explosion are not found in this range. It can be confirmed that there is no combustion and explosion in this range.

2, according to the 1090 ~ 1200m range of 0.5 ~ 1.5m thick coal dust, 1200m inward coal dust accumulation filled with the roadway, clean up the roadway, found that the roadway 1201 ~ 1280m section and the 1373-1381m section of the roof plate has obvious pressure, digging face front face 1381m before the large gangue as well as crushed coal blocked tightly, the top 2 meters of the roadway is the coal gangue, and the top 2 meters of the roadway is the coal gangue, and the top 2 meters of the roadway is the coal gangue, and the top of the roadway is the coal gangue.

The direct cause of the accident is that the construction unit did not pay attention to the changes in the geological structure and did not take any measures, and the construction mistakenly penetrated into the 19# coal seam, and the excavation gunning induced the coal and gas protrusion of the 19# coal seam and the high-concentration gas gushed out of the wellhead, which burned when it met the surface open fire.

(2) Indirect causes

1, the construction unit did not establish and improve the rules and regulations, safety management and site management chaos; construction did not prepare and formulate to prevent accidental penetration of the coal seam of the safety and technical measures, and did not drill to control the coal seam level to prevent accidental penetration of the coal seam; the serious shortage of special operators, in the case of no tile inspector to check the gas, unauthorized command of the construction of excavation and organization of artillery operations. The company's business is to provide the best possible service to its customers.

2, ring water coal mine outsourcing project supervision is not in place. The company has not strictly implemented the safety supervision responsibility of the construction unit.

3, the design unit along the 19 # coal seam (with coal and gas protrusion risk) bottom plate digging is not designed to prevent coal and gas protrusion precautionary measures and prevent accidental penetration of the coal seam of the coal exploration measures.

V. Lessons learned

(a) the handling of this accident, in accordance with the procedures for timely reporting, the start of the accident at all levels of emergency response plan in a timely manner, the development of the program out of the correct, the implementation of measures in place. Called the mine rescue team and public security firefighters *** with the rescue, collaborative action. First closed the shaft, closed 4 days later, the gas in the disaster area fell to the fire zone extinguishing parameters, 34 days to reach the extinguishing conditions, 45 days to fire zone sealing is completed, to restore the ventilation of the disaster area.

(C) construction unit to strengthen the safety management of coal mine construction projects, especially in the coal bedrock to do a good job of gas management, the implementation of the prevention and control measures of coal and gas protrusion. Comprehensively improve the construction of Karma personnel on the coal seam gas disaster awareness, and effectively implement the responsibility system, so that no construction without safety.

On Nov. 24, a major coal and gas outburst accident occurred in the Hexi mining area of the Pingshui coal mine of Pannan Coal Development Co. in Guizhou province, killing 23 people and injuring five others. Pannan Coal Development Co., Ltd. is a joint-stock enterprise, in which Guizhou Panjiang Investment Holding (Group) Co., Ltd. contributes 36%, Yankuang Guizhou Energy Chemical Co.
