I used to like watching the stories about the emergency department in film and television programs, and I felt that the doctors and nurses in the stories were so decisive, capable and charming that I envied them. I didn't expect that I was lucky enough to be an emergency nurse when I grew up many years later. From the beginning, I was in a hurry to gradually find the law, and I really realized that it was hard to be a good emergency nurse!
The emergency department is one of the most important windows in the hospital. My hospital is one of the top comprehensive third-class first-class hospitals in the province. It not only accepts normal acute and severe patients, but also accepts critically ill patients transferred from prefectures and counties. It is anxious, busy and complicated. In the face of the first diagnosis and rescue of critical emergency patients, emergency patients often have unclear diagnosis, unclear condition and rapid changes. If they are not handled properly, medical disputes will easily occur. However, patients and their families are prone to impatience, anxiety and fear. The emergency department is different from the ward. Medical staff have enough time to communicate with patients and their families, understand the needs of patients in time, and establish a good nurse-patient relationship. In the emergency department, patients and their families have not yet established a good sense of trust in a short time of contact with medical staff, and they are very sensitive to every subtle link of medical staff. The speech and behavior of medical staff will have a great impact on patients' psychology. As a result, the treatment behavior of medical staff is not understood, and patients and their families leave the illusion of "not being taken seriously" and "emergency is not urgent". Therefore, the more I feel that communication is the most easily overlooked and important thing in emergency.
Nightingale said: Nursing needs not only exquisite skills, but also art. First of all, we must learn to observe before we can be good at discovering the existence of problems, including observing the patient's condition, psychology, needs and family members. For example, some patients' family members are critical of nurses' behaviors, but they don't know all about their patients' fluids. For such family members, we should patiently remind them where to call a nurse when changing fluids. At the same time, we should also patrol the ward on time and observe the progress of fluids, so as to avoid conflicts with patients' family members to a great extent because the fluids are not changed in time. Secondly, we should learn to communicate. Mutual respect and trust between people are based on communication. For patients who come to the emergency department, we should actively ask and enthusiastically guide them. For example, we can help patients with mobility difficulties or help them to take them to the clinic by cart, measure their vital signs and give them to the nurses in the clinic; For patients who need and can go to the ward for treatment, we can point out the location of the ward in detail to prevent patients from running "wrongly". For patients who have been ill for a long time but are not critical, we can advise them to go to the outpatient clinic for medical treatment, and patiently and meticulously inform them of the location of the outpatient clinic and the time of medical treatment, which is convenient for patients to arrange. Although a smiling expression and a gesture of helping hands can't solve the physical pain of patients, it can quickly shorten the distance between nurses and patients and effectively meet the psychological needs of patients who need to be taken care of, thus facilitating us to carry out the following nursing work. Finally, through observation and communication, we can learn to judge, and have an accurate understanding and judgment of the patient's condition and needs, which is conducive to the development of medical staff's rescue work and also buys time for patients. For example, patients with trauma can know the location and severity of their injuries through observation and general inquiry, and find a doctor to treat them at the first time, and the registration procedures can be prepared.

Everyone is my teacher in the emergency department, and everything is my experience, which has taught me a lot, no matter how to be a person or do things. 3. Nurses' experience
Time flies, and the four-month training for emergency nurses is over. This formal theoretical study combined with clinical practice has gained more than any short-term academic activities in the past, which not only changed my understanding of emergency work, enriched my theory, but also improved my first-aid technical ability, and benefited a lot. The experience is as follows:
Four-month specialty. In fact, to really reach the level of emergency specialist nurses, my existing ability is still limited. The training method is divided into three stages: 4 weeks of theoretical study, 1 weeks of clinical practice and 2 weeks of summary report. In the first month, I took a theoretical course in the First Affiliated Hospital of Guangzhou Medical College. The contents were all about the new concepts and technologies of the development of emergency specialty knowledge, including nursing management, nursing technology, first aid coordination, symptom nursing, clinical evaluation technology, teaching training, nursing research and so on. The learning style of this specialist nurse training has given us a brand-new feeling and made our students particularly satisfied in two aspects. First, the course content is arranged reasonably, what we have learned is what we need in our work, and we can also learn how to deal with snake venom, bee sting, electric injury and so on. Emergency department is a general department. In our work, we often encounter such sudden situations and don't know how to deal with them. Therefore, the thoughtful training content of this course has given us a new understanding. Second, sufficient clinical practice allows us enough time to digest what we have learned. It can be seen that the provincial nursing society has made great efforts for our training. There are seven clinical practice base hospitals, and I was assigned to the First People's Hospital of Guangzhou and the First Affiliated Hospital of Sun Yat-sen University. During my study, these two hospitals gave me the impression that there are not many emergency patients in the First People's Hospital of Guangzhou, but the treatment process of emergency patients is optimized, convenient, centralized and the division of labor is clear, which is not only beneficial to the emergency treatment of critical patients but also reduces human resources, which is worth learning from. However, the First Affiliated Hospital of Sun Yat-sen University has a lot of patients, and their work is busy. Moreover, the patients are in critical condition, and they often encounter the rescue of major or special patients, which also makes me learn a lot of rescue experience. The layout of the rescue room is very reasonable, and the first-aid items are concentrated and clear at a glance. Moreover, the teachers of the First Affiliated Hospital of Sun Yat-sen University are very connotative. They have done excellent papers or scientific research and have strong teaching ability, which is also their highlight. Finally, we have to complete the paper report of nursing cases and group plans, make ppt and make a speech report, and pass the final pass of theory and skill operation before we can obtain the qualification certificate of emergency specialist nurses in Guangdong Province.
4. Experience of nurses
Experience of nurses working in emergency department is 2xx years, which is the fifth year that I have worked in emergency department and the last year that we have been groping in the emergency building. Next year, we will move to the outpatient building specially set aside for emergency department. At that time, our working environment and conditions will be greatly improved, and the business work of the department will develop faster.

Thanks to our concerted efforts, the number of outpatient and emergency services, the cumulative number of inpatients and emergency rescue in the emergency department have increased significantly compared with the same period of last year, which has delivered a large number of pathogens to various wards and auxiliary examination departments of the hospital, effectively promoted the growth of "two benefits" in various departments of the hospital and improved the enthusiasm of medical staff in the hospital.
First, strengthen ideological and political education and create a United and harmonious atmosphere
In the year of 2xx, all nursing staff in the emergency department earnestly studied various medical laws and regulations, consciously observed various rules and regulations of hospitals and departments, firmly established the service concept of "taking patients as the center", constantly improved their ideological and moral education, strived for civilized medical practice and high-quality service, and achieved the style of stressing minor matters and the principle of stressing major events. Second, strengthen professional technical training and improve emergency rescue ability
Nurses in emergency departments take "quality management year standard" and "national medical miles inspection standard" as the criterion, strengthen the construction of medical care quality in emergency departments, focus on the core system of medical care, and conscientiously implement various operating procedures and rescue workflow to prevent the occurrence of medical care adverse events such as mistakes and doctor-patient disputes. According to the training and assessment plan of the hospital and department system, the first-aid team members are regularly trained in business study and first-aid technology, which has formed a good learning atmosphere in the department, seriously studied business, communicated with each other and promoted each other, so that the department staff can master all kinds of rescue instruments and rescue technologies, such as cardiopulmonary resuscitation, mechanical ventilation, tracheal intubation, electric defibrillation, electric gastric lavage, ECG monitoring and other operating technologies, which further improves the first-aid ability of first-aid team members. Check, control, train, assess and supervise the medical and nursing quality of the department, strictly regulate the writing of medical and nursing documents, and carry out emergency rescue work in strict accordance with the relevant laws, regulations and medical behavior norms formulated by the Ministry of Health, so as to prevent medical errors and medical accidents before they happen, and put an end to careless thinking in the process of medical behavior.

In the process of "12" pre-hospital first aid and in-hospital first aid, we should evaluate the condition, pre-check, triage, and so on. Every link can be seamlessly connected, which effectively guarantees the life safety of residents in the jurisdiction.
Third, strengthen nursing management in departments and do a good job in hospital infection management in emergency work
In emergency rescue work, strengthen hospital infection management, effectively prevent and control hospital infection, improve medical care quality and ensure nursing safety. Implement hospital infection management rules and regulations and work norms, strictly implement operating procedures and work standards, prevent the spread of infectious diseases, do a good job in self-protection, disinfection and isolation, medical waste management, and prevent cross-infection in hospitals.
Fourth, strengthen the publicity of departments and establish the image of hospital window
The emergency department is an important window for hospitals to face the society, and its medical level and service quality directly affect and reflect the mental outlook and social image of hospitals. As an emergency nurse, it represents the image of the hospital in daily emergency work and pre-hospital emergency care. 5. Experience of nurses
Hemodialysis room is a special department, and the particularity of patients determines the special work of nurses in dialysis room. Half a year ago, when I was a layman, I thought that hemodialysis was just to treat patients with machines, and there was nothing for nurses when the machines were running, but when I became a hemodialysis nurse, I found that this job was far more complicated and arduous than I thought!
The first shift in the hemodialysis room is the morning shift, and it arrives at the post at 6: 3. Before the patient comes in, it is necessary to complete the pre-flushing of the patient's puncture needle, the preparation of anticoagulant, the installation and pre-flushing of the hemodialysis pipeline. All this work can't allow you to be slow. Every minute has its mission, and you feel the urgency of time. You must race against time and be accurate to ensure the orderly development of the day's work, and get on the computer. It is very important to monitor and judge the condition during dialysis. Nurses should monitor the patient's vital signs every hour, pay close attention to the changes of venous pressure and transmembrane pressure, and handle the alarm of the machine at any time to prevent sudden situations such as coagulation and membrane rupture during dialysis. Attention should also be paid to the observation and treatment of some crisis situations, such as hypotension, arrhythmia, hemolysis and air embolism.
Hemodialysis nurses can't leave the ward, because many situations can develop irretrievably in just a few minutes! It can be said that the work of a nurse in hemodialysis room is just like that of a rotating machine, and she can't slack off at all! Because more than 2 milliliters of blood are drawn from the patient's body every minute, hemodialysis nurses must always be vigilant until the patient gets off the plane smoothly after treatment.
For patients with chronic kidney disease in uremia stage, psychological care is difficult and important because of the long course of disease. At the same time, patients should be psychologically counseled, communicate with patients and their families, listen carefully to their feelings, and encourage patients to face life and diseases actively.
